内镜下注射矛头蝮蛇血凝酶治疗消化性溃疡出血的疗效及对血小板参数指标的影响

The curative effect of Agkistrodon hemagglutinin for injection in the treatment of peptic ulcer bleeding and the influence of platelet parameters

目的探究与分析内镜下注射矛头蝮蛇血凝酶治疗消化性溃疡出血的疗效及血小板参数指标的影响。方法采取随机数表法对自2016年3月~2021年3月收治的消化性溃疡出血患者120例分组,各60例,对照组给予传统方案治疗,观察组给予内镜下注射矛头蝮蛇血凝酶治疗,对比两组临床止血疗效、治疗前后pH值>6持续时间占总时间的百分率、手术相关指标以及血小板参数变化,同时观察治疗期间的不良反应。结果观察组与对照组相比临床止血总有效率较高,复发率较低,差异有统计学意义(P<0.05)。观察组治疗后与对照组治疗后相比pH值>6持续时间占总时间的百分率较高,BPC值较高、PCT值较高、MPV值较低,差异有统计学意义(P<0.05)。两组在治疗期间均无明显的凝血征象,且未见明显发热以及皮疹等不良反应。结论内镜下注射矛头蝮蛇血凝酶治疗消化性溃疡出血可获得更加显著的止血效果,改善pH值,调节血小板参数的同时降低了治疗后复发风险,且用药期间并未增加明显的并发症。

Objective To explore and analyze the curative effect of hemocoagulase injection of Agkistrodon viridis under endoscope in the treatment of peptic ulcer bleeding and the influence of platelet parameters.Methods A random number table was used to divide 120 patients with peptic ulcer hemorrhage admitted to our hospital from May 2018 to May 2020,60 cases in each group.The control group was treated with traditional regimen,and the observation group was given endoscope injection of Agkistrodon viridis for hemagglutinin treatment.The clinical hemostatic effects of the two groups were compared,the percentage of the duration of pH value>6 before and after treatment in the total time,the changes of operation-related indicators and platelet parameters,and the adverse reactions during the treatment period were observed.Results Compared with the control group,the observation group had a higher total clinical hemostasis rate and a lower recurrence rate.The difference was statistically significant(P<0.05).Compared with the control group after treatment,the observation group has a higher percentage of pH value>6 duration in the total time,higher BPC value,higher PCT value,and lower MPV value.The difference is statistically significant(P<0.05).During the treatment period,the two groups had no obvious signs of coagulation,and no obvious adverse reactions such as fever and rash were seen.Conclusion Endoscopic injection of Agkistrodon hemagglutinin to treat peptic ulcer bleeding can achieve a more prominent hemostatic effect,improve pH value,adjust platelet parameters and reduce the risk of recurrence after treatment,and increase the obvious complications during the medication.
