低剂量克拉霉素联合鼻内镜下鼻中隔矫正术及鼻窦开放术治疗慢性鼻-鼻窦炎的效果及患者ECP TIgE水平观察

Low-dose clarithromycin combined with endoscopic nasal septum correction and sinus opening in the treatment of chronic rhinosinusitis and the observation of patients’ ECP and TIgE levels

目的 研究低剂量克拉霉素联合鼻内镜下鼻中隔矫正术及鼻窦开放术治疗慢性鼻-鼻窦炎的效果及对患者嗜酸性粒细胞阳离子蛋白(ECP)、血清总免疫球蛋白E(TIgE)水平的影响。方法 回顾性分析2019年12月至2021年12月间我院收治的73例慢性鼻-鼻窦炎患者的临床资料,按治疗方式的不同将其分为常规组(实施鼻内镜下鼻中隔矫正术及鼻窦开放术治疗,共33例)和研究组(实施低剂量克拉霉素联合鼻内镜下鼻中隔矫正术及鼻窦开放术治疗,共40例)。对比两组治疗前后的ECP、TIgE、炎性因子水平[肿瘤坏死因子-a(TNF-a)、白细胞介素-6(IL-6)]、生活质量综合评定问卷(GQOLI-74)评分。结果治疗后,研究组的ECP、TIgE、TNF-a、IL-6水平均低于常规组(P<0.05);研究组的GQOLI-74评分高于常规组(P<0.05)。结论 通过低剂量克拉霉素联合鼻内镜下鼻中隔矫正术及鼻窦开放术治疗慢性鼻-鼻窦炎,能够有效降低患者的ECP、TIg E、TNF-a、IL-6水平,改善患者的生活质量,应用效果理想。

Objective To study the effect of low-dose clarithromycin combined with endoscopic nasal septum correction and sinus open surgery in the treatment of chronic rhinosinusitis and the effect on patients with eosinophil cationic protein(ECP) and serum total immunoglobulin E(TIgE) level of influence. Methods The clinical data of 73 patients with chronic rhinosinusitis admitted to our hospital from December 2019 to December 2021 were retrospectively analyzed, and they were divided into conventional groups according to different treatment methods(implementation of nasal septum correction under nasal endoscopy) Operative and open sinus surgery, 33 cases in total and the study group implementation of low-dose clarithromycin combined with endoscopic sinus septum correction and open sinus surgery, 40 cases in total. The levels of ECP, TIgE, inflammatory factors [tumor necrosis factor-a(TNF-a), interleukin-6(IL-6)] and the comprehensive quality of life assessment questionnaire(GQOLI-74) scores before and after treatment were compared between the two groups. Results After treatment,the ECP, TIgE, TNF-a, and IL-6 levels of the study group were lower than those of the conventional group(P<0.05);the GQOLI-74 score of the study group was higher than that of the conventional group(P<0.05). Conclusion Low-dose clarithromycin combined with endoscopic nasal septum correction and sinus open surgery for the treatment of chronic rhinosinusitis can effectively reduce the patient’s ECP, TIgE, TNF-a, IL-6 levels and improve the patient’s quality of life, The application effect is ideal.
