前庭康复练习治疗老年良性阵发性位置性眩晕远期的疗效观察

Long term effect of vestibular rehabilitation practice in the treatment of benign paroxysmal positional vertigo in the elderly

目的 为加强促进老年良性阵发性位置性眩晕(BPPV)患者的康复,分析采取前庭康复练习的远期疗效。方法 本次的研究对象是在我院接受治疗的老年BPPV患者,共有70例,均在2017年7月至2019年1月入我院。有1/2患者接受常规手法复位治疗,另外1/2患者再接受前庭康复练习治疗,分别纳入对照组与研究组。对两组患者的前庭症状指数(VSI)、眩晕残障程度量表(DHI)评分、Berg平衡量表(BBS)评分、基底动脉与椎动脉最大血流速度。结果 两组患者在治疗前的上述量表评分以及指标对比均不存在显著差异(P>0.05);在治疗后1个月的VSI、DHI、BBS不存在显著差异(P>0.05),但此时研究组的基底动脉与椎动脉最大血流速度高于对照组(P<0.05);在治疗后3个月,研究组的VSI、DHI低于对照组(P<0.05),BBS、基底动脉与椎动脉最大血流速度高于对照组(P<0.05)。结论 针对老年BPPV患者,采取前庭康复训练进行治疗,对提升患者的远期疗效有积极作用。

Objective To enhance the rehabilitation of elderly patients with benign paroxysmal positional vertigo(BPPV), and to analyze the long-term effect of vestibular rehabilitation practice. Methods The subjects of this study were 70 elderly BPPV patients who were treated in our hospital, all of which entered our hospital from July 2017 to January 2019. One in two patients received conventional manual reduction therapy, and another one in two patients received vestibular rehabilitation practice, which was included in the control group and the study group. The vestibular symptom index(VSI), vertigo disability scale(DHI), Berg Balance Scale(BBS), and the maximum blood flow velocity of basilar and vertebral arteries were evaluated. Results There was no significant difference in the scores and indexes of the two groups before treatment(P > 0.05);There was no significant difference in VSI, DHI and BBS in the first month after treatment(P > 0.05), but the maximum blood flow velocity of basilar artery and vertebral artery in the study group was higher than that of the control group(P < 0.05);The VSI and DHI of the study group were lower than that of the control group(P < 0.05), and the maximum blood flow velocity of BBS, basilar artery and vertebral artery was higher than that of the control group(P < 0.05). Conclusion The treatment of BPPV patients with vestibular rehabilitation is helpful to improve the long-term effect of the patients.
