鼻内窥镜下泪囊鼻腔吻合术联合丝裂霉素C治疗复发性慢性泪囊炎的疗效及安全性分析

Efficacy and safety analysis of dacryocystorhinostomy under nasal endoscope combined with mitomycin C in the treatment of recurrent chronic dacryocystitis

目的 探究复发性慢性泪囊炎患者实施鼻内窥镜下泪囊鼻腔吻合术+丝裂霉素C治疗的临床价值。方法 遴选时段2018年1月-2021年6月内24例复发性慢性泪囊炎患者,根据治疗计划不同分2组,单纯开展鼻内窥镜下泪囊鼻腔吻合术12例(记对照组),鼻内窥镜下手术配合丝裂霉素C治疗12例(记研究组),评估两组临床总疗效、手术并发症率、吻合口黏连情况并对比。结果 较对照组,研究组治疗总有效率较高;同时研究组远期吻合口面积大于对照组(P<0.05);研究组不良情况发生率8.33%明显低于对照组50.00%(P<0.05)。结论 实施鼻内窥镜下泪囊鼻腔吻合术治疗的复发性慢性泪囊炎患者,联合应用丝裂霉素C可取得显著的疗效,可预防吻合口瘢痕黏连,增强远期疗效,且治疗安全性有保障。

Objective To explore the clinical value of endoscopic dacryocystorhinostomy + mitomycin C treatment in patients with recurrent chronic dacryocystitis. Methods The selection period was from January 2018 to June 2021,and 24 patients with recurrent chronic dacryocystitis were divided into 2 groups according to different treatment plans,and 12 cases of dacryocystorhinostomy under nasal endoscopy were performed alone(note the control group), nasal endoscopic surgery combined with mitomycin C treatment of 12 cases(remember the study group), the total clinical efficacy, surgical complication rate, and anastomotic adhesion between the two groups were evaluated and compared.Results Compared with the control group, the total effective rate of treatment in the study group was higher;at the same time, the long-term anastomosis area in the study group was larger than that in the control group(P<0.05);the incidence of adverse events in the study group was significantly lower than that of the control group 50.00%(P<0.05). Conclusion In patients with recurrent chronic dacryocystitis treated by endoscopic dacryocystorhinostomy,the combined application of mitomycin C can achieve significant curative effect, which can prevent anastomotic scar adhesion and enhance long-term curative effect. The safety of treatment is guaranteed.
