利培酮联合氯氮平治疗难治性精神分裂症的临床疗效

Clinical efficacy of risperidone combined with clozapine in the treatment of refractory schizophrenia

目的观察利培酮联合氯氮平治疗难治性精神分裂症的临床疗效。方法选取2018年1月—2019年12月广东省江门市第三人民医院收治的难治性精神分裂症患者60例,根据随机数字表法分为试验组和对照组各30例。对照组患者实施氯氮平治疗,试验组患者实施利培酮联合氯氮平治疗,2组患者均治疗12周。比较2组患者临床疗效、治疗前后阳性和阴性综合征量表(PANSS)评分及不良反应。结果治疗12周后,试验组患者总有效率为96.67%,高于对照组的76.67%(χ^(2)=5.192,P=0.023);2组患者各项PANSS评分均较治疗前降低,且试验组均低于对照组(P<0.01);试验组患者不良反应总发生率为6.67%,低于对照组的26.67%(χ^(2)=4.320,P=0.038)。结论利培酮联合氯氮平治疗难治性精神分裂症的临床疗效较好,可明显降低患者PANSS评分,降低不良反应发生率,值得临床应用推广。

Objective To observe the clinical efficacy of risperidone combined with clozapine in the treatment of refractory schizophrenia.Methods From January 2018 to December 2019,60 patients with refractory schizophrenia admitted to the Jiangmen City Third People’s Hospital in Guangdong Province were selected and divided into the experimental group and the control group with 30 cases each according to the random number table method.Patients in the control group were treated with clozapine,patients in the experimental group were treated with risperidone combined with clozapine,and both groups were treated for 12 weeks.The clinical efficacy,positive and negative syndrome scale(PANSS)scores before and after treatment and adverse reactions of the two groups were compared.Results After 12 weeks of treatment,the total effective rate of patients in the experimental group was 96.67%,which was higher than 76.67% in the control group(χ^(2)=5.192,P=0.023);the PANSS scores of the two groups were lower than before treatment,and the experimental group was lower than the control group(P<0.01);the total incidence of adverse reactions in the experimental group was 6.67%,which was lower than the 26.67% in the control group(χ^(2)=4.320,P=0.038).Conclusion Risperidone combined with clozapine has a good clinical effect in the treatment of refractory schizophrenia.It can significantly reduce the PANSS score of patients and reduce the incidence of adverse reactions.It is worthy of clinical application and promotion.
