摘要
目的观察免疫肠内营养支持在胃癌患者术后的应用价值。方法选择2019年6月至2021年1月新乡市第二人民医院治疗的70例胃癌患者,根据随机数表法分为两组,每组35例。术后,研究组接受免疫肠内营养支持,对照组接受常规营养支持。营养干预前后检测两组患者转铁蛋白(TEN)、白蛋白(ALB)、前白蛋白(PA)、血管活性肠肽(VIP)、胃动素(MTL)、胃促生长素、降钙素原(PCT)、C反应蛋白(CRP)、CD4^(+)T淋巴细胞、CD8^(+)T淋巴细胞、免疫球蛋白A(IgA)水平,记录两组首次排气、排便时间及住院时间。结果干预前,两组各指标水平差异无统计学意义(P>0.05)。干预后,研究组患者TEN、ALB、PA、VIP、MTL、胃促生长素水平高于对照组(P<0.05),PCT、CRP水平低于对照组(P<0.05),CD4^(+)、IgA水平高于对照组(P<0.05),CD8^(+)水平低于对照组(P<0.05)。研究组首次排气、排便时间及住院时间短于对照组(P<0.05)。结论在胃癌患者术后进行免疫肠内营养支持干预,可有效改善患者营养状态,促进患者胃肠功能恢复,抑制炎症反应,提升免疫功能,促进患者康复。
Objective To observe the application value of immune enteral nutrition support in patients with gastric cancer after surgery.Methods A total of 70 gastric cancer patients treated in Xinxiang Second People’s Hospital from June 2019 to January 2021 were selected and divided into two groups according to the random number table method,with 35 cases in each group.After operation,the study group received immune enteral nutrition support,and the control group received routine nutritional support.Before and after nutritional intervention,transferrin(TEN),albumin(ALB),prealbumin(PA),vasoactive intestinal peptide(VIP),motilin(MTL),ghrelin,progenitor(PCT),C-reactive protein(CRP),CD4^(+)T lymphocytes,CD8^(+)T lymphocytes,and immunoglobulin A(IgA)levels were detected in the two groups of patients.The first exhaust and defecation time and hospital stay were recorded in the two groups.Results Before intervention,there was no statistical difference in the level of each index between the two groups(P>0.05).After intervention,the levels of TEN,ALB,PA,VIP,MTL and ghrelin in the study group were higher than those in the control group(P<0.05),the levels of PCT and CRP were lower than those in the control group(P<0.05),the levels of CD4^(+) and IgA were higher than those in the control group(P<0.05),and the level of CD8^(+) was lower than that in the control group(P<0.05).The first exhaust and defecation time and hospital stay in the study group were shorter than those in the control group(P<0.05).Conclusion Immune enteral nutrition support intervention in patients with gastric cancer after surgery can effectively improve the nutritional status of patients,promote the recovery of gastrointestinal function,inhibit inflammatory response,improve immune function,and promote the recovery of patients.
