芪苈强心胶囊联合坎地沙坦酯片治疗慢性心力衰竭的临床效果

Clinical effect of Qiliqiangxin capsule combined with candesartan cilexetil tablets in the treatment of chronic heart failure

目的 观察芪苈强心胶囊联合坎地沙坦酯片治疗慢性心力衰竭的临床效果及对血清N末端B型利钠肽原(NT-proBNP)和高敏肌钙蛋白T(hs-cTnT)水平的影响。方法 选取2018年7月—2020年6月临沂市中心医院收治的慢性心力衰竭患者92例,根据随机数字表法分为观察组和对照组,每组46例。在常规综合治疗基础上,对照组给予坎地沙坦酯片治疗,观察组在对照组基础上加用芪苈强心胶囊治疗,2组均持续治疗8周。比较2组患者临床疗效,治疗前后血清NT-proBNP、hs-cTnT水平和心功能指标[左心室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)]及不良反应。结果 治疗8周后,观察组总有效率为93.48%,高于对照组的76.09%(χ^(2)=5.392,P=0.020);2组血清NT-proBNP、hs-cTnT水平均明较治疗前降低,且观察组低于对照组(P均<0.01);2组LVEF较治疗前升高,LVEDD、LVESD较治疗缩小,且观察组升高或缩小幅度大于对照组(P均<0.01);2组患者实验室检查均未发现血液及肝肾功能异常,2组不良反应发生率比较差异无统计学意义(χ^(2)=0.211,P>0.05)。结论 芪苈强心胶囊联合坎地沙坦酯片治疗慢性心力衰竭效果显著,可改善患者心功能,降低血清NT-proBNP、hs-cTnT水平,且安全性高,值得临床推广应用。

Objective To observe the clinical effect of Qiliqiangxin capsule combined with candesartan cilexetil tablets in the treatment of chronic heart failure and influence on N-terminal B-type natriuretic peptide( NT-proBNP) and high-sensitivity cardiac troponin T( hs-cTnT) levels. Methods Ninety-two patients with chronic heart failure admitted to Linyi Central Hospital from July 2018 to June 2020 were divided into observation group and control group according to the random number table method,with 46 cases in each group. The patients in both groups were treated with routine treatment. The control group was given candesartan cilexetil tablets on the basis of the routine treatment,while the observation group was treated with Qiliqiangxin capsule on the basic of the control group. All the patients were treated for 8 weeks. To compare the clinical efficacy of the two groups of patients,serum NT-proBNP,hs-cTnT levels and cardiac function indexes before and after treatment[left ventricular ejection fraction( LVEF),left ventricular end diastolic diameter( LVEDD),left ventricular end systolic diameter( LVESD) ],the adverse reactions. Results After 8 weeks of treatment,the total effective rate in the observation group was 93. 48%,which was higher than 76. 09% in the control group( χ^(2)= 5. 392,P = 0. 020);Serum NT-proBNP and hs-cTnT levels in the 2 groups were lower than before treatment,and the observation group was lower than the control group( all P < 0. 01);The LVEF of the two groups was higher than before treatment,and LVEDD and LVESD were smaller than that of treatment,and the increase or decrease of the observation group was greater than that of the control group( all P < 0. 01);Laboratory examinations of the two groups of patients did not find abnormalities in blood,liver and kidney function,and there were no significant difference in the incidence of adverse reactions between the two groups( χ^(2)= 0. 211,P > 0. 05). Conclusion Qiliqiangxin capsule combined with candesartan cilexetil tablets has good clinical efficacy in the treatment of chronic heart failure,it can improve cardiac function and reduce serum NT-proBNP and hs-cTnT levels,which is higher safe,and worthy of clinical application.
