基于互联网平台的5A护理模式在0级糖尿病足患者护理中的应用

Application of 5A Nursing Model Based on Internet Platform in the Nursing of Patients with 0 grade Diabetic Foot

目的:探讨基于互联网平台的5A护理模式在0级糖尿病足患者护理中的应用效果。方法:选取2018年4月1日~2019年10月31日收治的82例0级糖尿病足患者为研究对象,按照随机数字表法分为对照组和观察组各41例,对照组采用常规护理,观察组采用基于互联网平台的5A护理模式,干预1年;比较两组糖尿病足护理相关知识得分、糖化血红蛋白水平、糖尿病足Wagner分级、生活质量[采用健康调查简表(SF-36)]、再入院情况及护理满意度。结果:重复测量方差分析,两组糖尿病足护理相关知识得分、糖化血红蛋白水平、生活质量评分及护理满意度得分的组间主效应、时间主效应和交互效应比较差异有统计学意义(P<0.05,P<0.01),两两比较,干预半年和1年后观察组上述指标优于对照组(P<0.05,P<0.01);护理1年,观察组再入院和足溃疡发生率均低于对照组(P<0.05,P<0.01),糖尿病足Wagner分级优于对照组(P<0.05)。结论:基于互联网平台的5A护理模式能够提高0级糖尿病足患者糖尿病足护理相关知识得分,增强对糖化血红蛋白水平的控制,延缓糖尿病足Wagner分级进展,改善生活质量,降低再入院和溃疡发生率,提升护理满意度。

Objective:To explore the application effect of the 5A nursing model based on the internet platform in the nursing of patients with 0 grade diabetic foot.Methods:During April 1,2018 to October 31,2019,82 cases of diabetic foot patients with Wagner 0 grade admitted were divided into the control group and the observation group by the random number table method with 41 cases in each group.The control group used routine care,and the observation group used 5A care based on the internet platform for 1 year of intervention.The scores of diabetic foot-related knowledge,glycosylated hemoglobin levels,Wagner classification of diabetic foot,scores of life quality by the mos item short from health survey(SF-36),readmissions and scores of nursing satisfaction were compared between the two groups of patients.Results:Repeated measures analysis of variance showed that the group main effect,time main effect and interaction effect of the two group patients′diabetic foot care-related knowledge scores,glycosylated hemoglobin levels and scores of life quality and nursing satisfaction were statistically significant(P<0.05),and pairwise comparison showed that the above indicators in the observation group were better than those in the control group after six months and 1 year intervention(P<0.05,P<0.01).After intervention,the incidence of readmission and foot ulcer in the observation group was lower than that in the control group(P<0.05,P<0.01),After one year,and the Wagner classification of diabetic foot was better than that in the control group(P<0.05).Conclusion:The 5A nursing model based on the internet platform can improve the diabetic foot-related knowledge scores in patients with 0 grade diabetic foot,enhance the control of glycosylated hemoglobin level,delay the progress of Wagner classification of diabetic foot,improve the life quality of patients,reduce the incidence rate of readmission and ulcer,and improve the nursing satisfaction.
