改良腮腺切除术治疗腮腺良性肿瘤的临床疗效

Clinical Efficacy of Modified Parotidectomy in the Treatment of Benign Parotid Tumors

目的探讨腮腺良性肿瘤应用改良腮腺切除术治疗的临床疗效及安全性。方法将2019年10月—2020年11月该院收治的78例腮腺良性肿瘤患者随机分为对照组(n=39)和研究组(n=39),对照组实施传统腮腺切除术治疗,研究组则接受改良腮腺切除术治疗,比较两组手术情况、生活质量评分及并发症发生率。结果两组术中出血量、手术时间、术后当天引流量比较差异无统计学意义(t=0.939、0.488、0.340,P>0.05);研究组手术切口长度(3.13±1.05)cm、住院时间(7.25±1.44)d明显低于对照组的(6.27±1.12)cm、(12.14±1.97)d,差异有统计学意义(t=12.773、12.515,P<0.05);研究组治疗后生理机能、社会职能、情感职能、精神健康方面的生活质量评分高于对照组,差异有统计学意义(t=4.653、3.276、2.779、4.215,P<0.05);研究组暂时性面瘫、面部瘢痕畸形、涎瘘、耳廓区麻木、面部神经损伤等总并发症发生率(17.95%)低于对照组(38.46%),差异有统计学意义(χ^(2)=4.052,P=0.044)。结论腮腺良性肿瘤应用改良腮腺切除术治疗的效果显著,可降低患者面部神经损伤率及并发症发生率,并有助于改善其术后生活质量。

Objective To investigate the clinical efficacy and safety of modified parotidectomy for benign parotid tumors.Methods A total of 78 patients with benign parotid tumors admitted to the hospital from October 2019 to November 2020 were randomly divided into control group(n=39)and study group(n=39).The control group received traditional parotidectomy,while the study group received modified parotidectomy.The surgical conditions,quality of life scores and complication rates were compared between the two groups.Results There was no statistically significant difference in intraoperative blood loss,operation time,and drainage volume on the day after operation between the two groups(t=0.939,0.488,0.340,P>0.05);the length of surgical incision(3.13±1.05)cm and the length of hospital stay(7.25±1.44)d in the study group were significantly lower than those in the control group(6.27±1.12)cm and(12.14±1.97)d,and the differences were statistically significant(t=12.773,12.515,P<0.05).After treatment,the quality of life scores in the study group in terms of physiological function,social function,emotional function and mental health were higher in the study group,and the difference was statistically significant(t=4.653,3.276,2.779,4.215,P<0.05);the total incidence of complications such as facial paralysis,facial scar deformity,salivary fistula,numbness of the auricle,facial nerve injury(17.95%)was lower than that of the control group(38.46%),and the difference was statistically significant(χ^(2)=4.052,P=0.044).Conclusion Modified parotidectomy is effective in the treatment of benign parotid tumors,which can reduce the rate of facial nerve injury and complications,and help improve the postoperative quality of life.
