雌二醇水平与帕金森病患者认知功能、抑郁、焦虑的相关性分析

Correlation analysis of estradiol level with cognitive function,depression and anxiety in patients with Parkinson's disease

目的探讨血清雌二醇(E2)水平与帕金森病(Parkinson disease,PD)患者认知功能、抑郁、焦虑等非运动症状的相关性。方法选择门诊或住院的PD患者60例(PD组)和年龄、性别、教育年限相匹配的健康对照者60例(对照组)。两组检测血清E2水平,检测前PD组停服所有抗PD药(如多巴丝肼、多巴胺受体激动剂)及降血压药等正在服用的药物至少12 h;老年快速认知筛查量表(QCSS-E)进行认知功能的评定,汉密尔顿抑郁量表(HAMD)和汉密尔顿焦虑量表(HAMA)评价PD组和对照组的抑郁和焦虑程度。结果PD组血清E2水平及HAMA、HAMD评分均明显高于对照组(P<0.05),QCSS-E评分明显低于对照组(P=0.001).女性PD患者血清E2水平低于男性PD患者(P=0.026);男、女PD患者QCSS-E、HAMA、HAMD评分比较差异均无统计学意义(P>0.05)。女性PD患者E2水平与QCSS-E评分呈正相关(r=0.713,P=0.047),与HAMA评分(r=-0.782,P=0.022)和HAMD评分(r=-0.769,P=0.043)呈负相关;男性PD患者E2水平与QCSS-E评分呈正相关(r=0.562,P=0.041),与HAMA评分(r=-0.190,P=0.086)和HAMD评分(r=-0.269,P=0.406)无相关性。结论女性PD患者E2水平与认知功能呈正相关、与抑郁及焦虑程度呈负相关。男性PD患者E2水平与认知功能呈正相关;与抑郁及焦虑程度无相关性。

Objective To investigate the correlation between estradiol(E2)level and cognitive function,depression and anxiety in Parkinson's disease(PD).Methods Sixty healthy subjects and 60 PD patients were selected as a control group and a PD group,respectively.The E2 level was detected at least 12 hours after the patients stopped taking the drug,such as levodopa benserazide,dopamine receptor agonists,blood pressure lowering drugs.The elderly Rapid Cognitive Screening Scale(QCSS-E)was used to evaluate the cognitive dysfunction.Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)were used to evaluate depression and anxiety symptoms.Results The E2 level,HAMA and HAMD scores in the PD group were significantly higher than those in the control group(P<0.05),and the QCSS-E score was significantly lower than that in the control group(P=0.001).The serum E2 level of female PD patients was lower than that of male PD patients(P=0.026).There was no significant differences in QCSS-E,HAMA and HAMD scores between male and female PD patients(P>0.05).E2 level in female PD patients was positively correlated with QCSS-E score(r=0.713,P=0.047),and negatively correlated with HAMA score(r=-0.782,P=0.022)and HAMD score(r=-0.769,P=0.043).E2 level in male PD patients was positively correlated with QCSS-E score(r=0.562,P=0.041),but not significantly associated with HAMA score(r=-0.190,P=0.086)or HAMD score(r=-0.269,P=0.406).Conclusions E2 level of female PD patients is positively correlated with cognitive function and negatively correlated with depression and anxiety.The level of E2 is positively correlated with cognitive function in male PD patients,but not associated with depression or anxiety.
