浅表性CD34阳性纤维母细胞性肿瘤一例

A case of superficial CD34-positive fibroblastic tumor

74岁女性,左大腿内侧结节伴红、肿、热、痛5年.皮损组织病理肿瘤位于真皮层,由梭形细胞、胖梭形细胞及多角形细胞组成,呈束状、编织状排列,部分瘤细胞呈上皮样或脂母细胞样,可见明显畸形或多形性核,核分裂像罕见.免疫组化染色示CD34和Vimentin弥漫阳性,INI-1表达无缺失,Ki-67阳性比例约1%.诊断:浅表性CD34阳性纤维母细胞性肿瘤.结节完整切除后随访6个月,未见复发.

A 74 years old female presented with a nodule on the inner side of the left thigh with redness,swelling,heat and pain for 5 years.Histopathology of the skin lesion showed that the tumor was located in the dermis with clear boundary and no capsule,which was composed of spindle cells,plump spindle cells and polygonal cells,arranged in interlacing bundles,as well as some epithelioid and lipoblastoid cells.There were obvious atypical and pleomorphic nuclei,but mitotic figures were rare.Immunohistochemical staining showed that,for the neoplastic cells,CD34 and vimentin were diffuse positive,there was no loss of INI-1 expression,and the positive proportion of Ki-67 was about 1%.Diagnosis:superficial CD34 positive fibroblastic tumor.The patient was followed up for 6 months after complete resection of the nodule,with no recurrence.
