高剂量尿激酶治疗血液透析患者自体动静脉内瘘血栓形成效果及安全性分析

Effect and Safety of High Dose Urokinase in the Treatment of Thrombus Formation of Autogenous Arteriovenous Fistula in Hemodialysis Patients

目的:分析高剂量尿激酶治疗血液透析患者自体动静脉内瘘血栓形成效果及安全性。方法:回顾性分析天津市宝坻区人民医院2019年1月—2020年1月收治的251例血液透析患者临床资料,将接受低剂量尿激酶(50万U)的126例血液透析患者纳入A组,将接受高剂量尿激酶(100万U)的125例血液透析患者纳入B组。比较分析两组患者自体动静脉内瘘血栓成功率及并发症。结果:B组溶栓效果优于A组,差异有统计学意义(χ^(2)=11.61,P<0.05);B组并发症发生率为8.00%,高于A组的3.97%,差异无统计学意义(χ^(2)=1.815,P>0.05)。结论:对血液透析患者使用高剂量尿激酶可以提高溶栓成功率,且不会增加溶栓风险,患者并发症发生率未见明显增加,安全性高。

Objective: To observe the effect and safety of high dose urokinase in the treatment of thrombus formation of autogenous arteriovenous fistula in hemodialysis patients. Methods: The clinical data of 195 hemodialysis patients in the hospital from January 2019 to January 2020 were analyzed retrospectively. 98 hemodialysis patients receiving low-dose urokinase(500 thousand U)were included in group A, and 97 hemodialysis patients receiving high-dose urokinase(1 million U) were included in group B. The success rate and complications of thrombus were compared between the two groups. Results: The thrombolytic effect of group B was better than that of group A, the difference was statistically significant(χ^(2)=11.61,P<0.05). The complication rate of group B was 8.00%, slightly higher than 3.97% of group A, but there was no statistically significant difference between the two groups(χ^(2)=1.815,P>0.05). Conclusion: The use of high-dose urokinase for hemodialysis patients can increase the success rate of thrombolysis without increasing the risk of thrombolysis. The incidence of complications in patients has not increased significantly, and the safety is high.
