沙参麦冬汤对肺炎患者血清炎症指标及免疫功能的影响研究

Effect of Shashen Maidong Decoction on the serum inflammatory indexes and immune function in patients with pneumonia

目的探讨沙参麦冬汤对肺炎患者血清炎症指标及免疫功能的影响。方法选取280例气阴两虚型肺炎患者,随机分为对照组与观察组,各140例。对照组予常规西药治疗,观察组在对照组的基础上服用沙参麦冬汤,连续治疗14 d后,比较2组临床疗效、中医证候积分、肺功能指标[用力肺活量(FVC)、呼气峰流速(PEFR)、最大通气量(MVV)]、血清炎症指标[可溶性尿激酶型纤溶酶原激活物受体(suPAR)、可溶性髓系细胞触发受体-1(sTREM-1)、白细胞介素-10(IL-10)]、免疫功能指标(CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+))及安全性。结果观察组治疗总有效率(93.57%,131/140)高于对照组(80.00%,112/140)(P<0.05)。治疗后,观察组中医证候积分低于对照组(P<0.05),FVC、PEFR及MVV高于对照组(P<0.05)。治疗后,观察组suPAR、sTREM-1、CD8^(+)表达水平低于对照组(P<0.05),IL-10、CD4^(+)表达水平及CD4^(+)/CD8^(+)高于对照组(P<0.05)。2组均无严重不良反应发生。结论沙参麦冬汤联合西药治疗肺炎临床疗效显著,可改善患者主要症状及肺功能,降低机体炎症反应、增强免疫功能。

Objective To investigate the effect of Shashen Maidong Decoction on the serum inflammatory indexes and immune function in patients with pneumonia.Methods A total of 280 pneumonia patients with the syndrome of deficiency of both qi and yin were selected and randomly divided into a control group and an observation group,with 140 cases in each.The control group was treated with conventional western medicine,while the observation group was treated with Shashen Maidong Decoction on the basis of the control group.After continuous treatment for 14 days,the clinical efficacy,TCM syndrome scores,lung function indexes[forced vital capacity(FVC),peak expiratory flow rate(PEFR),maximum ventilation volume(MVV)],serum inflammatory indexes[soluble urokinase type plasminogen activator receptor(suPAR),soluble myeloid cell triggered receptor-1(sTREM-1),interleukin-10(IL-10)],immune function indexes(CD4^(+),CD8^(+),CD4^(+)/CD8^(+))and the treatment safety were compared between the two groups.Results The total effective rate in the observation group(93.57%,131/140)was higher than that in the control group(80.00%,112/140)(P<0.05).After treatment,the TCM syndrome scores in the observation group were lower than those in the control group(P<0.05),while the scores of FVC,PEFR and MVV were higher than those in the control group(P<0.05).After treatment,the expression levels of suPAR,sTREM-1 and CD8^(+)in the observation group were lower than those in the control group(P<0.05),while the expression levels of IL-10,CD4^(+)and CD4^(+)/CD8^(+)in the observation group were higher than those in the control group(P<0.05).There were no serious adverse reactions in both groups.Conclusion Shashen Maidong Decoction combined with western medicine has a significant clinical effect in the treatment of pneumonia,which can improve the main symptoms and lung function,reduce the inflammatory response and enhance the immune function.
