香砂平胃散治疗小儿肠系膜淋巴结炎

Clinical efficacy of Xiangsha Pingwei Powder in the treatment of infantile mesenteric lymphadenitis

目的观察香砂平胃散治疗小儿肠系膜淋巴结炎的临床疗效。方法选择120例肠系膜淋巴结炎患儿作为研究对象,随机分为对照组与观察组,各60例。对照组予头孢克肟或者利巴韦林等对症治疗,观察组在对照组基础上口服香砂平胃散。比较2组临床疗效,中医证候恢复正常时间,治疗前后肠系膜淋巴结大小、T淋巴细胞亚群(CD3^(+)、CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+))水平变化及2个月后再次腹痛情况。结果观察组总有效率(90.0%,54/60)显著高于对照组(75.0%,45/60)(P<0.05);观察组症状体征恢复正常时间较对照组明显缩短(P<0.05);治疗后观察组肠系膜淋巴结横径、纵径均较对照组显著缩小(P<0.05);2组治疗后T淋巴细胞亚群水平均优于治疗前,且观察组优于对照组(P<0.05)。观察组再次腹痛率显著低于对照组(P<0.05)。结论香砂平胃散治疗肠系膜淋巴结炎效果显著,明显减轻患儿腹痛等临床症状,缩短症状消失时间,调节细胞免疫,减轻炎症反应,降低腹痛再发生率,安全可靠。

Objective To observe the clinical efficacy of Xiangsha Pingwei Powder in the treatment of infantile mesenteric lymphadenitis.Methods A total of 120 children with mesenteric lymphadenitis were selected as the research subjects,and randomly divided into a control group and an observation group,with 60 cases in each group.The control group was given a symptomatic treatment such as with cefixime or ribavirin,while the observation group was given Xiangsha Pingwei Powder orally on the basis of the control group.The clinical efficacy,time for the TCM syndrome to return to normal,changes in the mesenteric lymph node size,changes in the levels of T lymphocyte subsets(CD3^(+),CD4^(+),CD8^(+)and CD4^(+)/CD8^(+))before and after treatment,and the recurrence of abdominal pains after two months were compared between the two groups.Results The total effective rate(90.0%,54/60)in the observation group was significantly higher than that in the control group(75.0%,45/60)(P<0.05).The recovery time of symptoms and signs in the observation group was significantly shorter than that in the control group(P<0.05).After treatment,the transverse and longitudinal diameters of mesenteric lymph nodes in the observation group were significantly smaller than those in the control group(P<0.05).The levels of T lymphocyte subsets in the two groups after treatment were better than before,and the observation group was better than the control group(P<0.05).The recurrence rate of abdominal pains in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Xiangsha Pingwei Powder has a significant effect in the treatment of infantile mesenteric lymphadenitis,which can significantly relieve clinical symptoms such as abdominal pains,shorten the disappearance time of symptoms,regulate the cellular immunity,reduce inflammatory responses,and decrease the recurrence rate of abdominal pains.The treatment proves to be safe and reliable.
