消化内镜联合四联疗法治疗胃溃疡出血的临床效果观察

Clinical observation of digestive endoscopy combined with quadruple therapy in the treatment of gastric ulcer bleeding

目的探讨消化内镜联合四联疗法治疗胃溃疡出血的临床效果。方法选取2017年8月至2019年8月本院收治的92例胃溃疡出血患者作为研究对象,采用随机数字表法分为两组,每组46例。对照组采用四联疗法治疗,观察组采用消化内镜联合四联疗法治疗,比较两组溃疡愈合时间、止血时间、临床疗效、不良反应发生率和再出血率。结果观察组止血时间(18.35±6.28)h、溃疡愈合时间(21.76±5.39)d均短于对照组[(23.72±9.41)h、(25.19±5.48)d],差异有统计学意义(P<0.05);观察组治疗总有效率(93.48%)高于对照组(78.26%),差异有统计学意义(P<0.05);观察组不良事件发生率(13.04%)与对照组(8.70%)比较差异无统计学意义;观察组再出血率(10.87%)低于对照组(30.43%),差异有统计学意义(P<0.05)。结论消化内镜联合四联疗法治疗胃溃疡出血能有效缩短患者症状改善时间,提升疗效,降低再出血风险,具有较高的安全性。

Objective To explore the clinical effect of digestive endoscopy combined with quadruple therapy in the treatment of gastric ulcer bleeding.Methods 92 patients with gastric ulcer bleeding who were admitted to our hospital from August 2017 to August 2019 were selected as the research subjects,and they were divided into two groups by random number table method,with 46 cases in each group.The control group was treated with quadruple therapy,and the observation group was treated with digestive endoscopy combined with quadruple therapy.The ulcer healing time,hemostasis time,clinical efficacy,incidence of adverse reactions and rebleeding rates were compared between the two groups.Results The hemostatic time(18.35±6.28)h and the ulcer healing time(21.76±5.39)d in the observation group were shorter than those in the control group([23.72±9.41]h,[25.19±5.48])d,and the differences were statistically significant(P<0.05).The total effective rate of the observation group(93.48%)was higher than that of the control group(78.26%),the difference was statistically significant(P<0.05);there was no significant difference in the incidence of adverse events between the observation group(13.04%)and the control group(8.70%);the rebleeding rate in the observation group(10.87%)was lower than that in the control group(30.43%),and the difference was statistically significant(P<0.05).Conclusion Gastrointestinal endoscopy combined with quadruple therapy in the treatment of gastric ulcer bleeding can effectively shorten the symptom improvement time,improve the curative effect,and reduce the risk of rebleeding,with good safety.
