加味补肾活血汤内服外敷在股骨粗隆间骨折患者全髋关节置换术后治疗中的应用

Application of Jiawei Bushen Huoxue decoction by oral and external application in the treatment of patients with intertrochanteric fracture after total hip replacement

目的探讨加味补肾活血汤内服外敷在股骨粗隆间骨折(Intertrochanteric fracture of femur,IFF)患者全髋关节置换术(Total hip replacement,THR)后治疗中的应用价值。方法选择2020年3月至2021年3月本院收治的行THR的IFF患者80例作为研究对象,随机分为观察组和对照组各40例。对照组术后接受常规治疗措施,观察组在对照组基础上采用加味补肾活血汤内服外敷治疗,治疗两个月,评估患者临床疗效,于治疗前、治疗两个月后,比较两组骨代谢指标[骨碱性磷酸酶(BALP)、甲状旁腺激素(PTH)]、髋关节功能及疼痛评分[髋关节功能评分(Harris)、视觉模拟评分法(VAS)]。结果治疗两个月后,观察组总有效率高于对照组,差异有统计学意义(P<0.05);治疗两个月后,两组BALP低于治疗前,PTH高于治疗前,且观察组BALP低于对照组,PTH高于对照组,差异有统计学意义(P<0.05);治疗两个月后,两组Harris评分高于治疗前,VAS评分低于治疗前,且观察组Harris评分高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05)。结论加味补肾活血汤内服外敷能够有效改善行THR治疗的IFF患者骨代谢,提高患者髋关节功能,减轻疼痛水平。

Objective To investigate the application value of Jiawei Bushen Huoxue decoction by oral and external application in the treatment of patients with intertrochanteric fracture(IFF)after total hip replacement(THR).Methods A total of 80 IFF patients who underwent THR in our hospital from March 2020 to March 2021 were selected as the research subjects and were divided into an observation group and a control group,with 40 cases in each group.The control group received routine treatment after operation,and the observation group was treated with Jiawei Bushen Huoxue decoction by oral and external application on the basis of the control group.After two months of treatment,the clinical efficacy of patients was evaluated.Before and two months after treatment,the bone metabolism indexes[bone alkaline phosphatase(BALP),parathyroid hormone(PTH)],hip joint function and pain score[hip joint function score(Harris),visual analogue scale(VAS)]were compared between the two groups.Results After two months of treatment,the total effective rate of the observation group was higher than that of the control group,with a statistical significant difference(P<0.05).After two months of treatment,BALP was lower and PTH was higher in both groups than those before treatment,and BALP was lower and PTH was higher in the observation group than those in the control group,with statistical significant differences(P<0.05).After two months of treatment,the Harris score was higher and the VAS score was lower in both groups than those before treatment,and the Harris score was higher,the VAS score was lower in the observation group than those in the control group,with statistical significant differences(P<0.05).Conclusion Jiawei Bushen Huoxue decoction by oral and external application can effectively improve bone metabolism,hip joint function and relieve pain in patients with IFF after THR treatment.
