基于医联体平台构建医院-基层-家庭癌症三级护理模式的可行性分析

Feasibility analysis of hospital-basic-family cancer tertiary care model based on medical integration platform

目的:分析基于医联体平台构建癌症医院-基层-家庭三级护理模式的可行性。方法:采用《肿瘤的防治及护理常识》问卷对保定市莲池区三个社区的1856名社区健康居民及832例癌症患者进行问卷调查,并对调查结果进行分析。结果:调查的结果显示,在癌症防治知识的了解渠道方面,有78.2%的社区居民了解癌症防治知识的渠道是通过媒体报道,有46.2%的癌症患者了解癌症防治知识的渠道是通过医院宣传;在护理方面,被调查人群均不甚了解癌症的护理,其中有59.0%的癌症患者及其家属对癌症护理的相关知识了解较少,但相较于健康人群,癌症患者对癌症高危人群、癌前病变、癌症的症状及基本日常护理方面知识的了解程度较高;在治疗方面,有76.0%的癌症患者不排斥中医治疗,有63.9%的癌症患者正在接受中西医结合治疗,有90.9%的癌症患者能接受带癌长期生存。结论:建议在三级诊疗制度下,基于医联体平台构建医院-基层-家庭三级护理模式,加大对癌症防治知识的宣传,以提高社区居民及癌症患者对癌症防治知识的认知水平,提升护理服务的质量。

Objective:To analyze the feasibility of constructing cancer hospital-basic-family tertiary nursing model based on medical integration platform.Methods:1856 healthy residents and 832 cancer patients in three communities in Lianchi district of Baoding city were investigated by using the questionnaire of“Common sense of Cancer prevention and Nursing”,and the results were analyzed.Results:The results of the survey showed that 78.2%of the community residents knew the knowledge of cancer prevention and control through media reports,46.2%of the cancer patients knew the knowledge of cancer prevention and control through hospital publicity;In terms of nursing,the investigated population did not know much about cancer nursing,among which 59.0%of cancer patients and their families had little knowledge about cancer nursing,but compared with the healthy population,cancer patients had a higher understanding of cancer high-risk groups,precancerous lesions,cancer symptoms and basic daily care knowledge.In terms of treatment,76.0%of cancer patients do not reject TCM treatment,63.9%of cancer patients are receiving integrated traditional Chinese and Western medicine treatment,and 90.9%of cancer patients can accept long-term survival with cancer.Conclusion:It is suggested that under the three-level diagnosis and treatment system,the three-level nursing mode of hospital-grass-root and family should be constructed based on the medical alliance platform,and the publicity of cancer prevention and treatment knowledge should be strengthened to improve the cognition level of cancer prevention and treatment knowledge of community residents and cancer patients and improve the quality of nursing services.
