病理因素对高血压弦脉脉图变化规律研究

摘要 目的:应用不同高血压动物实验模型,对比研究血管与血黏度对脉象脉图不同影响,分析获得影响因素作用与脉图变化规律的关联关系。方法:对健康比格犬在麻醉状态先后注入1%去甲肾上腺素液和6%中分子右旋糖酐注射液,观测血压,记录脉图、心电图、心脏彩超等指标,分析两种因素对脉图影响规律。结果:1.去甲肾上腺素对主波幅值(H;)影响不大,对重搏波幅值(H;)有缩减作用,对降中峡幅值(H;)、重搏波幅值+降中峡幅值(HH;)有增加作用趋势。2.脉通对全部观察值呈缩减作用。3.舒张期室间隔厚度(diastolic interventricular septum,IVSd)与收缩期室间隔厚度(systolic interventricular septum,IVSs)、左心室舒张内径(left ventricular diastolic diameter,LVIDd)、左心室收缩内径 (left ventricular systolic diameter,LVIDs)、舒张期左室后壁厚度(diastolic left ventricular posterior wall,LVPWd)、收缩期左室后壁厚度(systolic left ventricular posterior wall,LVPWs)随去甲肾上腺素加量呈降低趋势,应用脉通后达到新高。结论:1.血液黏度增加对心脏做功的抑制较为显著,可能是气虚血瘀证的病理基础,心电图各导联随用药后血压升高呈上升趋势;2.脉搏强弱与心脏做功耗能不同步,与血压变化也不一定同步;3,降中峡与重搏波的变化对血黏度与脉管拘急有一定表达作用。 Objective: Using different hypertensive animal experimental models to compare the effects of blood vessels and blood viscosity on different pulse condition and diagram, and to analyze the correlation between the effects of influencing factors and the changing pat -terns of pulse diagrams. Methods: Healthy beagles were injected with 1% norepinephrine solution and 6% middle molecule dextran injec -tion successively in anesthesia state. Blood pressure changes were observed, pulse diagram, electrocardiogram, echocardiography and other indexes were recorded, and the influence rules of these two factors on pulse diagram were analyzed. Results: 1. Norepinephrine had little effect on H;, but had decreasing effect on H;, and increasing effect on H;and HH;(H;+H;);2. Middle molecule dextran had a decreasing effect on all observed values. 3. VSD and IVSS, IVIDD, IVIDS, LVPWD, LVPWS showed a decreasing trend with the addition of nore -pinephrine, and reached a new high level after applying middle molecule dextran. Conclusion: 1. Increased blood viscosity significantly inhibited cardiac work, which may be the pathological basis of qi deficiency and blood stasis syndrome. Every lead of the EKG increased with the increase of blood pressure;2. Pulse strength is not synchronized with energy consumption of cardiac work, nor is it necessarily synchronized with blood pressure change;3. The changes of dicrotic notch and hard wave had certain expression effects on blood viscosity and vascular contracture.
