多层螺旋CT在食管异物中的诊断价值 被引量:1

摘要 目的:探讨多层螺旋CT(MSCT)及其后处理技术在食管异物中的诊断价值。方法:回顾性分析2017年1月-2020年12月临床拟诊为食管异物的100例患者临床及影像学资料,100例患者均行X线吞钡挂棉检查、常规MSCT及其多平面重组(MPR)、曲面重建(CPR)最大密度投影(MIP)、容积再现(VR)等后处理,对这两种放射学诊断结果的差异进行统计学分析。结果:100例患者中临床确诊80例食管异物,X线吞钡挂棉诊断的敏感度、特异度、准确度、阳性预测值、阴性预测值、漏诊率、误诊率分别为83.75%(67/80)、65%(13/20)、80%(80/100)、87.50%(70/80)、43.48%(10/23)、16.25%(13/80)、35%(7/20);MSCT及其后处理技术诊断的敏感度、特异度、准确度、阳性预测值、阴性预测值、漏诊率、误诊率分别为97.50%(78/80)、100%(20/20)、96%(96/100)、100%(78/78)、90.91%、2.50%(2/80)、0%。两种检查方法的各项差异均有统计学意义(P<0.05)。结论:MSCT结合后处理技术较X线吞钡挂棉检查对食管异物的检出率高、准确性高,值得应用。 Objective:To investigate the diagnostic value of multi-slice spiral CT(MSCT)and its post-treatment in esophageal foreign body.Methods:The clinical and imaging data of 100 patients clinically suspected to be esophageal foreign bodies from January 2017 to December 2020 were retrospectively analyzed.All 100 patients underwent X-ray barium swallowing examination,conventional MSCT and its multi-plane recombination(MPR),curved planar reformation(CPR),maximum density projection(MIP),volume representation(VR)and other post-treatments.The difference of the two radiological diagnosis results was analyzed statistically.Results:Among the 100 patients,80 cases of esophageal foreign bodies were clinically diagnosed.The sensitivity,specificity,accuracy,positive predictive value,negative predictive value,misseddiagnosis,misdiagnosis rate of X-ray barium swallowing examination was 83.75%(67/80)、65%(13/20)、80%(80/100)、87.50%(70/80)、43.48%(10/23)、16.25%(13/80)、35%(7/20),respectively.The sensitivity,specificity,accuracy missed diagnosis,misdiagnosis rate of MSCT and its post-treatment was 97.50%(78/80)、100%(20/20)、96%(96/100)、2.50%(2/80)、0%,respectively.There were statistically significant differences between the two methods(P<0.05).Conclusion:MSCT combined with its post-treatment is more accurate and accurate than X-ray barium swallowing examination in the detection of esophageal foreign body.
