卢秉久教授基于“肝体阴用阳”辨治原发性胆汁性胆管炎

Clinical experience of Professor LU Bingjiu in diagnosing and treating primary biliary cholangitis based on the academic theory of“liver belongs to Yin in physique and Yang in function”

原发性胆汁性胆管炎又称原发性胆汁性肝硬化,是一种慢性胆汁淤积性自身免疫性肝病。本病以肝内胆汁淤积,循环血液中出现抗线粒体抗体或特异性抗核抗体为主要特征,伴随慢性非化脓性、肉芽肿性、淋巴细胞小胆管胆管炎。卢秉久教授从事肝胆病治疗近40年,对原发性胆汁性胆管炎的治疗颇具成效。文章详细阐述卢秉久教授诊疗该病的学术思想,将其列见于下:卢师认为,肝体阴而用阳,原发性胆汁性胆管炎之为病,不外乎肝体不足,肝用失和,即体用失调。故治疗本病清肝与柔肝举药同施,疏肝与养血体用同治。临证治法上可归纳为养血柔肝、温肾养肝、疏肝理气、健脾益气、消瘕利水等方法。

Primary biliary cholangitis,also known as primary biliary cirrhosis,is a chronic cholestatic autoimmune liver disease.This disease is characterized by intrahepatic cholestasis and the appearance of anti-mitochondrial antibodies or specific anti-nuclear antidodies in the circulating blood,accompanied by chronic non-suppurative,granulomatous,lymphocytic cholangitis.Professor LU Bingjiu has been engaged in the treatment of hepatobiliary diseases for nearly 40 years,and has many effective methods for treating primary biliary cholangitis.Now Professor LU’s experience in treating the disease was detailedly listed in this article.Mr.LU believes that the academic theory of liver belongs to Yin in physique and Yang in function(肝体阴而用阳),and the cause of the onset of primary biliary cholangitis is the lack of liver body and the disharmony of liver function,that is,the disorder of liver body and liver function.In the treatment of this disease,Professor LU soothed the liver and softened the liver at the same time,and treating the liver disease and nourishing the body at the same time.The clinical treatment methods can be summarized as nourishing blood and softening the liver,warming the kidney and nourishing the liver,soothing the liver and regulating Qi(气),invigorating the spleen and replenishing Qi,and eliminating gallbladder and diuresis.
