摘要
目的探讨血清激活素A(ACT-A)与类风湿关节炎(RA)的相关性。方法收集2020年1月~8月于我院住院确诊的71例RA患者作为病例组,同期40例健康体检者作为对照组。采用ELISA法测定病例组和对照组ACT-A水平,并根据RA患者ACT-A上下四分位数(25%,75%)分为ACT-A低水平组(<27.54 ng/mL)、中等水平组(27.54~39.14 ng/mL)及高水平组(>39.14 ng/mL),比较三个亚组间临床资料及实验室指标,并分析ACT-A与RA患者临床资料及实验室指标之间相关性。结果RA患者血清ACT-A水平明显高于健康患者,差异具有统计学意义(P<0.05)。关节压痛数、DAS28在ACT-A高水平组明显高于中等水平组及低水平组,关节肿胀数在高水平组高于低水平组,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,RA患者血清ACT-A水平与关节压痛个数、关节肿胀个数、DAS28呈正相关(r分别为0.678、0.452、0.575,P<0.05),与年龄、病程、类风湿因子(RF)、抗环瓜氨酸肽抗体(CCP)抗体、抗突变型瓜氨酸波形蛋白(MCV)抗体、C反应蛋白(CRP)、红细胞沉降速度(ESR)、IgA、IgM、IgG、IgE、补体C3、补体C4无相关性(P>0.05)。结论血清ACT-A在RA患者中高表达,与RA患者关节压痛数、关节肿胀数、DAS28呈正相关,可作为RA病情活动的参考指标。
Objective To investigate the relationship between the expression of sero-activin A and rheumatoid arthritis.Methods Seventy-one RA patients admitted to The Affiliated Hospital of Southwest Medical University from January 2020 to August 2020 were enrolled in case group.Forty subjects were enrolled to healthy control group in the same period.ELISA was used to detect the expression of ACT-A in both group.According to upper and lower quartile of ACT-A,RA patients were divided into low level group(<27.54ng/m),medium level group(27.54~39.14ng/ml)and high level group(>39.14ng/ml).Clinical data and laboratory indicators were compared in the 3 groups.The relationship between ACT-A and clinical data/laboratory indicators were analyzed.Results Sero-activin A was significant higher in RA patients compared to health controls(P<0.05),tenderness joint count(TJC),disease active score 28(DAS28)is higher in high level group compared to low level group and medium level group,swollen joint count(SJC)in high level group is higher than low level group.Pearson correlation analysis showed that serum ACT-A was related to TJC,SJC and DAS28,but there was no significant relationship with age,disease duration,rematoid factor(RF),anti-CCP antibody,anti-MCV antibody,C-reactive protein,erythrocyte sedimentation rate,IgA,IgM,IgG,complement C3,complement C4.Conclusion Serum ACT-A is significant higher in RA patients which is positively correlate to TJC,SJC and DAS28,and it may be a reference index for RA disease activity.
