探讨MRI多参数成像技术在前列腺癌患者诊断中的应用价值 被引量:6

摘要 目的探讨MRI多参数成像技术在前列腺癌(PCA)患者诊断中的应用价值。方法选取2020年1月至2021年2月信阳市中心医院收治的疑似PCA患者85例,均接受MRI的T2加权成像(T2WI)、动态增强(DCE)与弥散加权成像(DWI)扫描,术后病理学检查显示45例为PCA患者,40例为前列腺良性病变患者,比较两组感兴趣区转运常数(Ktrans)、速率常数(Kep)、血管外细胞外间隙体积百分数(Ve),同时分析MRI多参数成像技术单独或联合应用诊断PCA的效能。结果在b=50 s/mm^(2)与800 s/mm^(2)时,与良性病变组比较,PCA组的信号强度值与ADC值均明显了低(P<0.05);相较于良性病变组,PCA组DCE上Ktrans、Kep与Ve更高(P<0.05);MRI诊断PCA时T2WI联合DWI的特异度最高,且准确度与灵敏度也较高。结论MRI多参数成像技术在PCA诊断中具备较好的应用效果,特别是T2WI联合DWI,可使诊断效能明显提高。 Objective To explore the application value of MRI multi-parameter imaging in the diagnosis of prostate cancer(PCA)patients.Methods A total of 85 suspected PCA patients were selected,and all underwent T2-weighted imaging(T2 WI),dynamic enhancement(DCE)and diffusion weighted imaging(DWI)scans of MRI.Postoperative pathological examination showed that 45 patients were PCA patients and 40 patients were benign prostate lesions.The region of interest transport constant(Ktrans),rate constant(KEP)and extracellular space volume percentage(VE)were compared between the two groups,and the efficacy of multi-parameter MRI imaging alone or in combination in the diagnosis of PCA was analyzed.Results When b=50 S/mm^(2)and 800 S/mm^(2),the signal intensity value and ADC value of PCA group were lower than those of benign lesion group(P<0.05).Compared with the benign lesion group,the DCE values of Ktrans,Kep and Ve in PCA group were higher(P<0.05).In MRI diagnosis of PCA,T2 WI combined with DWI showed the highest specificity,accuracy and sensitivity.Conclusion MRI multi-parameter imaging technology has a good application effect in the diagnosis of PCA,especially T2 WI combined with DWI can significantly improve the diagnostic efficiency.
