儿童皮肤毛发移行疹5例临床分析 被引量:1

摘要 目的探讨儿童皮肤毛发移行疹(cutaneous pili migrans,CPM)的临床特点、治疗及预后,以提高皮肤科医生对CPM的认识,减少临床工作中的漏诊、误诊。方法回顾性分析本科诊治的5例儿童CPM的临床资料,并复习相关文献。结果患儿中男1例,女4例,皮损均位于足部,其中2例位于足底,2例位于踇趾,1例位于趾甲下,主要表现为皮下/甲下黑色线状异物,可伴有移行轨迹,不痒,可有轻微疼痛,去除黑色毛干后痊愈。结论儿童CPM主要发生在暴露于更高压力的足底和足趾的凸起区域,亦可发生在甲下,皮肤镜和显微镜检查有助于鉴别。本病预后较好,用镊子夹出异物后5~7 d皮损可完全消退,未见复发。 Objective To elucidate the clinical characteristics,treatment and prognosis of cutaneous pili migrans(CPM)in children,so as to improve the clinicians′understanding of CPM and reduce missed diagnosis and misdiagnosis in clinical practice.Methods The clinical data of 5 children with CPM were analyzed retrospectively,and the relevant literature was reviewed.Results There were 1 male and 4 female patients.All of the lesions were located on the feet,2 on the soles,2 on the toes and 1 under the toenail.The black thread-like foreign bodies seen under the skin or the nail were main clinical manifestations,which could be accompanied by migration trajectory,with slight pain,but without itching.After the black hair shaft was removed,the lesions were cured soon.Conclusion CPM in children is mainly located in the protruding area of the soles and toes exposed to higher pressure,and it can also occur under the nail.Dermoscopy and microscopy are helpful in differential diagnosis,and the prognosis of the disease is good.The skin lesions can completely disappear 5 to 7 days after the foreign body is clipped by tweezers,and no recurrence is found.
