接受和未接受HAART治疗的中青年HIV感染者颈动脉超声表现对比分析

Comparative Analysis of Carotid Ultrasound Manifestations in Young and Middle-Aged HIV Patients with and without HAART

目的超声检查评价接受和未接受高效抗逆转录治疗(highly active anti-retroviral therapy,HAART)对中青年HIV感染者颈动脉弹性及内-中膜厚度(inter-media thickness,IMT)的影响。方法将2015年5月-2020年12月在四川省凉山州第一人民医院收治的中青年HIV感染者50例分为A组(接受HAART治疗)25例、B组(未接受HAART治疗)25例,另设C组(对照组)为25例健康中青年,三组均进行颈动脉超声测量顺应性系数(CC)、僵硬度(β)、脉搏波传导速度(PWV)、IMT,多组间比较采用单因素方差分析。结果A组与C组、B组与C组的CC、β、PWV差异具有统计学意义(P<0.05),A组与B组的CC、β、PWV差异无统计学意义(P>0.05);三组间IMT的差异无统计学意义(P>0.05)。结论无症状期的中青年HIV感染者接受与未接受HAART治疗对颈动脉弹性无显著影响。无症状期的中青年HIV感染者在IMT未增厚情况下,已出现血管弹性功能较健康人群下降,应尽早对HIV感染人群动脉弹性进行筛查评估和积极干预。

Objective To evaluate the effect of highly active anti-retroviral therapy(HAART)treatment on carotid artery elasticity and inter-media thickness(IMT)in young and middle-aged HIV patients.Methods From May 2015 to December 2020,50 young and middle-aged HIV-infected patients admitted to the First People′s Hospital of Liangshan Prefecture in Sichuan Province were divided into group A(HAART treatment,n=25),group B(no HAART treatment,n=25).Another 25 healthy young and middle-aged people were enrolled as group C(control group).Compliance coefficient(CC),stiffness(β),pulse wave velocity(PWV),and IMT in three groups were measured by carotid artery ultrasound examination,and one-way analysis of variance was used for comparison among the three groups.Results There were significant differences in CC,βand PWV between group A and group C,and between group B and group C(P<0.05).There were no significant differences in CC,βand PWV between group A and group B(P>0.05).There were no significant differences in IMT between group A and group B,between group A and group C,and between group B and group C(P>0.05).Conclusion HAART treatment has no significant effect on carotid elasticity in young and middle-aged HIV infected patients.Without the increase of IMT,the young and middle-aged people with HIV infection in asymptomatic period has decreased vascular elastic function compared with healthy people.Therefore,it is necessary to screen and evaluate the arterial elasticity of HIV-infected people as early as possible.
