乳腺癌化疗患者并发带状疱疹的分析研究

Analysis of herpes zoster in breast cancer patients undergoing chemotherapy

目的:探讨乳腺癌化疗患者人群中带状疱疹的发病情况及发病特点。方法:回顾性分析2014年1月至2019年12月期间诊断为乳腺癌并接受化疗患者的临床资料,随访并统计该患者群中在化疗中及化疗后并发带状疱疹的病例,分析入组患者的一般资料、化疗相关的指标及带状疱疹发病的特点。结果:在随访的487例乳腺癌化疗的患者中,有17例患者并发带状疱疹,发病率为3.49%;其中辅助化疗14例,晚期患者3例。76.47%的患者接受了8疗程联合化疗,82.35%的患者曾在化疗期间出现3~4度粒细胞减少。腰腹部是最常出现带状疱疹症状的部位,占64.71%。阿昔洛韦是最常用的抗病毒药物。有超过一半的患者出现水疱合并神经性疼痛,平均病程为25.3 d。结论:乳腺癌化疗患者会增加带状疱疹的发病几率,提高医生对带状疱疹的认识并做好患者宣教,对该病的早诊早治具有积极的作用。

Objective:To investigate the incidence and characteristics of herpes zoster in breast cancer patients undergoing chemotherapy.Methods:The patients diagnosed with breast cancer and received chemotherapy in our department from January 2014 to December 2019 were retrospectively analyzed.The patients diagnosed with herpes zoster during and after chemotherapy were followed up.The general information of patients with herpes zoster,the indicators related to chemotherapy and the characteristics of herpes zoster were analyzed.Results:Among 487 patients who had been followed up,17 cases were diagnosed with herpes zoster.The incidence rate was 3.49%.14 cases were adjuvant chemotherapy and 3 cases were advanced breast cancer.76.47%of the patients received 8 courses of combined chemotherapy.82.35%of the patients had grade 3-4 neutropenia during chemotherapy.Acyclovir is the most commonly used antiviral drug.More than half of the patients had blisters and neuropathic pain,with an average course of 25.30 days.Conclusion:Breast cancer patients with chemotherapy will increase the incidence of herpes zoster infection.Raising the awareness of doctors and good education for patients could play an important role in the early diagnosis and treatment of the disease.
