廉江地区高危人乳头瘤病毒感染特点及相关影响因素分析

Characteristics and influencing factors of high-risk human papillomavirus infection in Lianjiang area

目的:探讨廉江地区高危人乳头瘤病毒(HR-HPV)感染特点及相关HR-HPV感染影响因素。方法:选取2021年2月~2021年9月在廉江市中医院和廉江市人民医院行宫颈癌筛查项目的2000例廉江地区女性为研究对象,采用HPV分型检测试剂盒检测HR-HPV的亚型。根据是否感染HR-HPV将研究对象分为感染组(264例)和对照组(1736例)。采用多因素Logistic回归分析HR-HPV感染的影响因素。结果:检出HR-HPV的18个亚型,检出率为13.2%(264例);总检出频次为315次,检出频次前三位依次为HPV-52(50次,15.9%)、HPV-16(35次,11.1%)、HPV-58(31次,9.8%)。HR-HPV的单一感染构成比为79.2%(209例)。多因素Logistic回归分析显示,初次性生活年龄<20岁、性伴侣≥2个、吸烟、性伴侣包皮过长、居住在农村均是HR-HPV感染的独立危险因素,而避孕套避孕和同房前后清洗则是独立保护因素(P<0.05)。结论:廉江地区的HR-HPV亚型和HR-HPV感染的影响因素众多,以HPV-52和单一感染最为常见,应加快九价疫苗的接种。

Objective:To investigate the characteristics of high-risk human papillomavirus(HR-HPV)infection and its influencing factors in Lianjiang area.Methods:From February 2021 to September 2021,2000 women who underwent cervical cancer screening in Lianjiang Hospital of Traditional Chinese Medicine and Lianjiang People's Hospital were selected as subjects.HPV genotyping kit was used to detect the subtypes of HR-HPV.The subjects were divided into the infection group(264 cases)and the control group(1736 cases)according to whether they were infected with HR-HPV.Multivariate Logistic regression was used to analyze the influencing factors of HR-HPV infection.Results:All 18 subtypes of HR-HPV were detected,and the total detection rate was 13.2%(264 cases).The total detection frequency was 315 times.The top three detection frequencies were HPV-52(50 times,15.9%),HPV-16(35 times,11.1%)and HPV-58(31 times,9.8%).The proportion of single infection of HR-HPV was 79.2%(209 cases).Multivariate logistic regression analysis showed that the age of first sexual life<20 years old,sexual partners≥2,smoking,long foreskin of sexual partners and living in rural areas were independent risk factors for HR-HPV infection,while condom contraception and cleaning before and after rooming were independent protective factors(P<0.05).Conclusion:There are many HR-HPV subtypes and influencing factors of HR-HPV infection in Lianjiang area,among which HPV-52 and single infection are the most common.The vaccination of nine valent vaccine should be accelerated.
