综合性护理干预对直肠癌术后永久性结肠造口患者护理效果及生活质量的研究

Study of comprehensive nursing intervention on nursing effect and quality of life of patients with permanent colostomy after rectal cancer surgery

目的:分析综合性护理干预措施对永久性结肠造口患者术后造口并发症及生活质量的影响。方法:选取本院胃肠外科2019年1月~2020年5月住院的直肠癌患者60例作为研究对象,采用随机数字表将患者分为两组,观察组(n=30)和对照组(n=30);观察组给予综合性护理干预方案;对照组给予传统的护理模式。观察并比较两组患者术后并发症发生情况(造口水肿、出血、黏膜分离、周围皮炎)、生活质量(总体健康、躯体功能、情绪功能、认知功能、社会功能、角色功能)以及患者出院3个月后应用生活质量量表(EORTC QLQ-C30)调查对护理人员工作的满意度。结果:观察组患者术后肠造口并发症的发生率显著低于对照组,生活质量方面显著优于对照组(P<0.001);患者对护理人员服务的满意度观察组(93.33%)高于对照组(46.67%)(P<0.001)。结论:对于结肠造口患者实施综合性护理干预能有效降低患者肠造口并发症的发生率,提高患者术后的生存质量及患者对护理人员的护理满意度,值得推广。

Objective:To analyze the effect of comprehensive nursing intervention on postoperative colostomy complications and quality of life in patients with permanent colostomy.Methods:A total of 60 patients with rectal cancer admitted to our hospital from January 2019 to May 2020 were selected as the research subjects.The patients were divided into two groups by random number table,the experimental group(n=30)and the control group(n=30).The experimental group was given comprehensive nursing intervention program;the control group was given traditional nursing mode.The incidence of postoperative complications(stoma edema,bleeding,mucosal separation,peripheral dermatitis),quality of life(general health,physical function,emotional function,cognitive function,social function,role function)and the satisfaction of nurses with quality of life scale(EORTC QLQ-C30)were observed and compared between the two groups.Results:The incidence of postoperative colostomy complications in the experimental group was significantly lower than that in the control group,and the quality of life was significantly better than that in the control group(P<0.001).The satisfaction of nursing staff in the experimental group(93.33%)was higher than that in the control group(46.67%)(P<0.001).Conclusion:Comprehensive nursing intervention can effectively reduce the incidence of colostomy complications in patients with colostomy,improve the postoperative quality of life and nursing satisfaction,which is worthy of promotion.
