摘要
目的探究同型半胱氨酸(homocysteine,Hcy)水平与2型糖尿病(type 2 diabetes,T2DM)患者冠心病(coronary heart disease,CHD)严重程度的相关性。方法选取2019年1月—2021年2月收治的100例T2DM合并CHD患者作为本研究的观察组,另选取同期就诊的100例T2DM患者(未合并CHD)作为对照组,比较两组患者的同型半胱氨酸(homocysteine,Hcy)水平,并对其相关性进行分析。结果观察组患者的Hcy水平明显高于对照组患者(P<0.05);不同冠状动脉病变支数患者的Hcy水平存在显著差异,其中多支病变组患者的Hcy水平明显高于单支病变组(P<0.05);不同Hcy水平患者的Gensini评分存在显著差异,其中Hcy重度升高组与Hcy轻度升高组患者的高Gensini评分比例明显大于Hcy正常组,且以Hcy重度升高组患者最为显著(P<0.05)。经多元Logistic回归分析,Hcy、糖化血红蛋白(glycosylated hemoglobin,HbA1c)均是导致T2DM患者CHD加重的危险因素。结论Hcy水平与T2DM患者的CHD严重程度存在显著相关性,其水平升高是导致冠心病发生及发展的重要危险因素。
Objective To explore the correlation between the level of homocysteine(Hcy)and the severity of coronary heart disease(CHD)in patients with type 2 diabetes(T2DM).Methods A total of 100patients with T2DM and CHD admitted from January 2019 to February 2021 were selected as the observation group of this study,and 100patients with T2DM(without CHD)who visited the same period were selected as the control group,and the two groups were compared.The Hcy level and its correlation are analyzed.Results The Hcy level of the observation group was significantly higher than that of the control group(P<0.05);and the Hcy index of patients with different coronary artery disease numbers was significantly different.Among them,the Hcy index of patients in the multivessel disease group was significantly higher than that of the single-vessel disease group(P<0.05).In addition,the Gensini scores of patients with different Hcy levels were significantly different.Among them,the proportion of high Gensini scores in the severely elevated Hcy group and the slightly elevated Hcy group was significantly greater than that of the Hcy normal group,and the patients in the severely elevated Hcy group were the most significant(P<0.05).With coronary heart disease as the dependent variable,multivariate logistic regression analysis found that Hcy and HbA1c are risk factors for diabetic coronary heart disease.Conclusion There was a significant correlation between the Hcy level and the severity of CHD in T2DM patients,and its elevated level leads to an important risk factor for the occurrence and development of coronary heart disease.
