基于导航模板的遮挡性病灶模型的曲针穿刺效果分析

摘要 目的:研究在模拟遮挡性病灶的体外模型上,利用导航模板进行曲针穿刺的可行性。方法:设计3款模拟遮挡性病灶的体外模型,按照靶区遮挡程度从小到大分为模型A、模型B、模型C。其中模型A和模型B采用直针穿刺(straight needle puncture,SNP)、曲针穿刺(curved needle puncture,CNP)和导航模板辅助曲针穿刺(navigation template guided curved needle puncture,简称“T-CNP”)3种穿刺模式,而模型C只采用CNP和T-CNP 2种穿刺模式。分析3种穿刺模式安全操作(损伤次数为0次)、便捷操作(CT扫描次数≤3次)的占比及其与靶区遮挡程度的关系。采用SPSS 13.0软件进行统计学分析。结果:在3种穿刺模式中,CNP的安全操作和便捷操作占比均最低。随着靶区遮挡程度的增大,从模型A到模型B,SNP和T-CNP的安全操作和便捷操作占比均呈下降趋势,但T-CNP的占比小于SNP。从模型A到模型B再到模型C,T-CNP的安全操作和便捷操作占比呈减速下降趋势,数值变动较小,穿刺结果较为稳定。结论:在导航模板的辅助下,曲针穿刺的临床可行性(安全性和便捷性)得到提高,可实现对遮挡性病灶的穿刺。 Objective To investigate the feasibility of applying navigating template to enhancing the effectiveness of the curved needle puncture for in vitro hidden lesion models.Methods Three in vitro models were designed to simulate hidden lesions,and were divided into models A,B and C according to the degree of obscuring.Models A and B used straight needle puncture(SNP),curved needle puncture(CNP)and navigation template guided curved needle puncture(T-CNP),whereas model C used only two puncture modes of CNP and T-CNP.The percentages of safe operation(0 times of injury)and convenient operation(3 times or less of CT scans)of the three puncture modes and their relationship with the degree of obscuring were analyzed,and SPSS 13.0 software was used for statistical analysis.Results CNP had the lowest percentages of safe and convenient operations in the three puncture modes.The percentages of safe and convenient operations for both SNP and T-CNP decreased from model A to model B as the degree of obscuring increased,while the percentages for T-CNP were lower than those for SNP.From model A to model B to model C the percentages of safe and convenient operations for T-CNP showed a decelerating decline,with low variation in values and stable puncture results.Conclusions The clinical feasibility(safety and convenience)of curved needle puncture is improved with the aid of the navigation template,which enables the puncture of hidden lesions.[Chinese Medical Equipment Journal,2022,43(3):46-50]
