糖尿病导致下肢动脉血管闭塞后再通修复重建对患者血流动力学的影响

Effect of Recanalization and Reconstruction on Hemodynamics in Patients After Lower Extremity Arterial Occlusion Caused by Diabetes

目的:分析糖尿病导致下肢动脉血管闭塞后再通修复重建对患者血流动力学的影响。方法:选取北京市化工职业病防治院治疗的86例糖尿病导致下肢动脉血管闭塞患者,分成对照组和研究组,各43例患者,对照组采取相应的综合治疗措施,研究组在此基础上联合经皮腔血管成形术治疗,对比两组患者的治疗效果、血流动力学指标。结果:研究组治疗有效率高于对照组,差异有统计学意义(P<0.05)。研究组患者治疗后血容量(BV)、达峰时间(TTP)、平均通过时间(MTT)、血流量(BF)各项血流动力学指标均优于对照组,差异有统计学意义(P<0.05)。治疗后研究组患者踝肱指数(ABI)低于对照组,差异有统计学意义(P<0.05)。结论:糖尿病导致下肢动脉血管闭塞患者应用经皮腔血管成形术治疗的临床疗效显著,可以有效改善患者血流动力学指标,值得推广使用。

Objective: To analyze the effect of recanalization and reconstruction on hemodynamics in patients after lower extremity arterial occlusion caused by diabetes. Methods: Eighty-six patients with diabetic arterial occlusions of lower extremities were selected from the Beijing Prevention and Treatment Hospital of Occupational Disease for Chemical Industry. The patients were divided into the study group and the control group, with 43 patients in each group. The control group was treated with corresponding comprehensive treatment measures. On the basis of the control group, the study group was treated with percutaneous transluminal angioplasty. The therapeutic effect and hemodynamic indexes of patients in the two groups were compared. Results:The effective treatment rate in the study group was higher than that in the control group, with significant difference(P<0.05). After treatment, the hemodynamic indexes including blood volume(BV), time to peak(TTP), mean transit time(MTT), and blood flow(BF) in the study group were better than those in the control group, and the differences were statistically significant(P<0.05). After treatment, the ankle-brachial index of the study group was lower than that of the control group, and the differences were statistically significant(P<0.05). Conclusion: The clinical efficacy of percutaneous transluminal angioplasty in patients with diabetic arterial occlusions of lower extremities is significant, which can effectively improve the hemodynamic indexes of patients and is worth promoting.
