房颤患者左心房/左心耳血栓检测中运用CMR的临床效果

The Clinical Effect of CMR in the Detection of Left Atrium/Left Atrial Appendage Thrombosis in Patients with Atrial Fibrillation

目的:观察房颤患者左心房/左心耳血栓检测中运用心脏磁共振(CMR)的临床效果。方法:选取2020年4月~2021年4月某院收治的86例心房震颤患者为研究对象,按照检测方式的不同将患者随机等分为对照组和研究组各43例,对照组采用经食道超声诊断,研究组采用CMR诊断,以上级医院病理分析作为检测金标准。对比两组患者的左心房前后径、左心房左右径、左心耳面积变化率等检测指标以及检查结果和病理结果的对比。结果:两组持续性房颤患者的左心房前后径和左右径明显大于阵发性房颤患者,持续性房颤患者左心耳面积变化率小于阵发性房颤患者;组间对比,研究组患者左心房前后径和左右径明显大于对照组,左心耳面积变化率小于对照组(P<0.05);同时,研究组敏感度、特异度、准确率均高于对照组(P<0.05)。结论:房颤患者左心房/左心耳血栓检测中运用CMR具有较好的临床应用效果,检查结果较优,检测指标更为详细,值得推广应用。

Objective:To observe the clinical effect of cardiac magnetic resonance(CMR)in the detection of left atrium/auricula sinistra thrombosis in patients with atrial fibrillation.Methods:A total of 86 patients with atrial fibrillation admitted to a hospital from April 2020 to April 2021 were selected as the research objects,and the patients were randomly divided into two groups according to the different detection methods.43 patients in the control group received transesophageal ultrasound diagnosis,and 43 patients in the study group received CMR diagnosis,and pathological analysis of higher-level hospitals was used as the gold standard for detection.The left atrium anterior and posterior diameter,left atrium left and right diameter,left atrial appendage area change rate and other detection indicators were compared between the two groups,and the examination results of the two groups were compared with pathological results.Results:The left atrium anterior and posterior diameter,and left atrium left and right diameter of the two groups of patients with persistent atrial fibrillation were significantly larger than those of patients with paroxysmal atrial fibrillation,and the rate of change in the area of the left atrial appendage was smaller than that of patients with paroxysmal atrial fibrillation;The left atrium anterior-posterior diameter and the left-right diameter of the study group were significantly larger than those of the control group,and the change rate of the left atrial appendage area was smaller than that of the control group(P<0.05).At the same time,the sensitivity,specificity and accuracy of the study group were higher than those of the control group(P<0.05).Conclusion:The application of CMR in the detection of left atrium/left atrial appendage thrombosis in patients with atrial fibrillation has good clinical effect,with better examination results and better detailed detection indicators,which is worthy of promotion and application.
