摘要
目的观察三维超声自由解剖切面(Omniview)联合容积对比成像(VCI)技术(O+V技术)测量肛提肌裂孔面积的价值。方法回顾性分析283名接受经会阴实时三维盆底超声检查女性,对比观察不同医师(医师1、医师2)采用O+V技术及立体渲染模式(R模式)测量最大瓦尔萨尔瓦状态下最小肛提肌裂孔面积所用时长,并采用组内相关系数(ICC)评估观察者内、观察者间及不同方法间测量结果的一致性,评价O+V技术用于测量肛提肌裂孔面积的价值。结果医师1及2采用O+V技术测量肛提肌裂孔面积所用时长均明显低于R模式(P均<0.01)。医师2采用O+V技术与R模式测量肛提肌裂孔面积的一致性均好(ICC均>0.75),2次O+V技术测量结果的一致性好(ICC>0.75);医师1、2采用O+V技术测量结果的一致性均好(ICC均>0.75),医师1采用O+V技术与医师2采用R模式测量结果的一致性亦好(ICC>0.75)。结论三维超声O+V技术用于测量肛提肌裂孔面积耗时短、准确度佳,值得临床推广及应用。
Objective To observe the value of three-dimensional ultrasound Omniview combined with volume contrast imaging(VCI)technique(O+V technique)for measuring the area of levator ani hiatus.Methods Data of 283 women who underwent transperineal real-time three-dimensional pelvic floor ultrasound were retrospectively analyzed.The time consuming of measuring the minimum area of levator ani hiatus under the maximum Valsalva maneuver with O+V technique and multi-volume rendering mode(R mode)by different doctors(doctor 1 and doctor 2)were observed and compared,and the intra-class correlation coefficient(ICC)was used to evaluate both inter-and intra-observer and intra-method variabilities,and the value of O+V technique was analyzed.Results The time consuming for measuring the area of levator ani hiatus with O+V technique by doctor 1 and 2 were both less than those with R mode(both P<0.01).The consistencies of the area of levator ani hiatus measured with O+V technique and R mode by doctor 2 were good(both ICC>0.75),as well as measured twice with O+V technique by doctor 2(ICC>0.75),measured with O+V technique by doctor 1 and 2(both ICC>0.75)and measured with O+V technique by doctor 1 and with R mode by doctor 2(ICC>0.75).Conclusion Three-dimensional ultrasound O+V technique was less time consuming with good accuracy for measuring the area of levator ani hiatus,hence being worthy of clinical promotion and application.
