痰热清联合肺泡灌洗治疗小儿重症肺炎的临床疗效

Clinical curative effect and safety of intravenous drip of Tanreqing injection combined with alveolar lavage for severe pneumonia in children

目的对痰热清联合肺泡灌洗治疗小儿重症肺炎的临床效果及安全性进行分析,为小儿重症肺炎的治疗提供理论依据。方法选取2019年1月至2020年1月在陕西省延安大学附属医院儿科接受诊治的72例重症肺炎患儿,随机分为对照组(采用肺泡灌洗等对症支持,36例)和治疗组(在对照组基础上加用痰热清注射液治疗,36例)。对比分析两组的临床疗效、白细胞计数、体温恢复正常的时间、肺部啰音消失时间、咳嗽咯痰基本消失时间、氧合指数及不良反应发生情况。结果治疗后,治疗组的临床有效率显著高于对照组(91.67%vs.72.22%,χ^(2)=4.599,P<0.05)。治疗组的白细胞计数、体温恢复正常时间、肺部啰音消失时间、咳嗽咯痰基本消失时间、氧合指数均显著低于对照组,差异均有统计学意义(t值分别为3.152、3.542、2.924、2.865、4.251,P<0.05)。对照组与治疗组不良反应发生率差异无统计学意义(χ^(2)=0.727,P>0.05)。结论痰热清联合肺泡灌洗治疗小儿重症肺炎的临床效果优于单用肺泡灌洗及对症治疗,中西医结合疗法是小儿重症肺炎新的治疗方向。

Objective To analyze clinical curative effect and safety of intravenous drip of Tanreqing injection combined with alveolar lavage for severe pneumonia in children,so as to provide a theoretical basis for treatment of the disease.Methods A total of 72 children with severe pneumonia who were diagnosed and treated in Department of Pediatrics,The Affiliated Hospital of Yan’an University,Shaanxi province from January 2019 to January 2020 were randomly divided into control group(symptomatic and supporting therapies including alveolar lavage were given,36 cases)and treatment group(intravenous drip of Tanreqing injection on basis of the symptomatic and supporting therapies as in the control group,36 cases).The clinical curative effect,white blood cell count(WBC),time of body temperature returning to normal range,lung rales disappearance time,cough and expectoration approximate disappearance time,oxygenation index and adverse reactions of the children were analyzed and compared between the two groups.Results After treatment,the clinical effective rate in the treatment group was significantly higher than that in the control group(91.67%vs.72.22%,χ^(2)=4.599,P<0.05).WBC,time of body temperature returning to normal range,lung rales disappearance time,cough and expectoration approximate disappearance time and oxygenation index of the children in the treatment group were all significantly lower than those in the control group(t=3.152,3.542,2.924,2.865 and 4.251 respectively,all P<0.05).While in incidence of adverse reactions in the children,there was no statistically significant difference between the two groups(χ^(2)=0.727,P>0.05).Conclusion The clinical curative effect of Tanreqing injection combined with alveolar lavage for severe pneumonia in children is better than that of alveolar lavage alone and symptomatic treatment.Combination of Chinese traditional medicine and Western medicine is a new treatment direction for severe pneumonia in children.
