内镜下低温等离子射频消融术治疗早期声门型喉癌的效果观察

Clinical Effect of Endoscopic Low-temperature Plasma Radiofrequency Ablation in the Treatment of Early Glottic Laryngeal Carcinoma

目的探究内镜下行低温等离子射频消融术治疗早期声门型喉癌的临床效果。方法选择100例早期声门型喉癌患者,采用随机数字表法分为2组,各50例。对照组行常规喉裂开术,研究组采取内镜下低温等离子射频消融术。对比2组围术期指标、手术前后语音指标及并发症。结果研究组手术时间为(10.34±1.53)min,短于对照组的(53.20±4.78)min;术中出血量为(9.25±0.64)ml,少于对照组的(80.10±8.56)ml;住院时间为(4.70±0.83)d,短于对照组的(7.16±1.22)d,差异均有统计学意义(P<0.05)。研究组治疗后振幅微扰为(2.61±0.33)%、基频微扰为(1.14±0.12)%,均小于对照组的(4.29±0.37)%、(1.29±0.18)%;谐噪比为(23.29±3.58)dB,大于对照组的(20.16±3.27)dB,差异均有统计学意义(P<0.05)。研究组术后并发症发生率为4.00%,低于对照组的22.00%,有统计学差异(P<0.05)。结论对早期声门型喉癌患者于内镜下行低温等离子射频消融术治疗具有创伤小、术后恢复快等优势,有助于患者的语言功能恢复,安全性高,疗效确切。

Objective To explore the clinical effect of endoscopic descending cryogenic plasma radiofrequency ablation for early glotal laryngeal carcinoma.Methods 100 patients with early glottal laryngeal cancer were divided into 2 groups and 50 cases in each.The control group performed routine laryngeal slit,and the study group performed endoscopic cryogenic plasma radiofrequency ablation.Compared perioperative indicators,speech indicators before and after surgery and complications of the 2 groups.Results The surgical duration of the study group was(10.34±1.53)min,shorter than(53.20±4.78)min of the control group;postoperative bleeding of the study group was(9.25±0.64)ml,less than(80.10±8.56)ml of the control group;hospital stay was(4.70±0.83)d,shorter than(7.16±1.22)d of the control group(P<0.05).Treatment amplitude perturbation was(2.61±0.33)%and base frequency perturbation(1.14±0.12)%in the study group,all less than(4.29±0.37)%and(1.29±0.18)%of the control group;the harmonic noise ratio of the study group was(23.29±3.58)dB,greater than the control group(20.16±3.27)dB,differences were significant(P<0.05).The incidence of postoperative complications in the study group was 4.00%,lower than 22.00%in the control group,with statistical differences(P<0.05).Conclusion The early glotal laryngeal cancer patients with cryogenic plasma radiofrequency ablation treatment have low trauma and rapid postoperative recovery,which helps to restore language function,high safety and precise efficacy.
