摘要
目的探讨鸢尾素(Irisin)水平与急性肾损伤(AKI)的相关性。方法选取2019年12月至2020年12月在成都医学院第一附属医院住院的AKI患者(试验组)及健康体检者(对照组)各30例作为研究对象,检测血清Irisin、人中性粒细胞明胶酶关联脂质运载蛋白(NGAL)、肾功能及尿Irisin、尿NGAL等相关指标。结果试验组患者血清Irisin水平较对照组明显升高(P<0.05),两组尿Irisin水平差异无统计学意义(P>0.05)。血清Irisin与血肌酐(Scr)呈正相关(P<0.05),与估算肾小球滤过率(eGFR)、血清白蛋白(ALB)呈负相关(P<0.05),eGFR、ALB和血清NGAL为影响AKI患者血清Irisin水平的独立影响因素(P<0.05)。血清Irisin水平的升高与AKI的发生相关,其对AKI的最佳诊断界点值为0.536μg/L,灵敏度为83.3%,特异性为90.0%,曲线下面积为0.899(P<0.05)。结论AKI患者血清Irisin水平与AKI的发生呈正相关,主要受eGFR、ALB和血清NGAL的影响,血清Irisin有望成为诊断AKI的潜在生物学标志物。
Objective To investigate the correlation between Irisin level and acute kidney injury(AKI).Methods AKI patients hospitalized in the First Affiliated Hospital of Chengdu Medical College from December 2019 to December 2020 were selected and included in the experimental group(n=30),and healthy subjects were selected and included in the control group(n=30).Serum Irisin,human neutrophil gelatinase-associated lipocalin(NGAL),renal function,urine Irisin,urine NGAL and other related indicators were detected.Results Serum Irisin level in experimental group was significantly higher than that in control group(P<0.05),but there was no significant difference in urine Irisin level between the two groups(P>0.05).Serum Irisin was positively correlated with serum creatinine(Scr)(P<0.05),and was negatively correlated with estimated glomerular filtration rate(eGFR)and serum albumin(ALB)(P<0.05).eGFR,ALB and serum NGAL were independent influencing factors of serum Irisin level(P<0.05).The increase of serum Irisin level was associated with the occurrence of AKI.The optimal diagnostic cutoff value of serum Irisin for AKI was 0.536μg/L,the sensitivity was 83.3%,the specificity was 90%,and the area under the curve was 0.899(P<0.05).Conclusion Serum Irisin level in AKI patients is positively correlated with the occurrence of AKI,which is mainly affected by eGFR,ALB and serum NGAL.Serum Irisin is expected to be a potential biomarker for the diagnosis of AKI.
