云南省ART患者服药依从性及影响因素调查

Medication adherence and influencing factors of HIV/AIDS patients receiving antiretroviral therapy in Yunnan Province

目的探索云南省接受ART的HIV/AIDS患者服药依从性的特点及影响因素。方法2020年6-12月,将调查问卷发布在YNART微信小程序上,采用方便抽样的方法,邀请绑定小程序的患者自主参与服药依从性评估。结果共收到有效问卷1001份,自评Morisky服药依从性得分(6.6±1.5)分,其中,依从性好者279人(27.9%),依从性中等436人(43.6%),依从性差者286人(28.6%)。患者服药依从性与年龄(P<0.05)、服药次数(P<0.001)、感染途径(P<0.05)、既往是否脱失(P<0.01)有关,50岁及以上者、一天服药一次者依从性较好,吸毒感染者、既往脱失过的患者依从性较差。结论影响HIV/AIDS患者ART服药依从性的因素是多方面的,应该采用综合的依从性评估手段获得患者服药依从性的真实情况,建议将依从性不良者纳入艾滋病个案管理,提早干预,预防其病毒载量反弹及脱失。

Objective To explore the characteristics and influencing factors of medication adherence of HIV/AIDS patients receiving antiretroviral therapy in Yunnan Province.Methods The questionnaire was published in the YNART WeChat Mini Program,and the patients bound to it participated in the Morisky Medication Adherence Scale survey voluntarily.Results From June to December 2020,1001 valid questionnaires were received.The self-rated Morisky medication adherence score was 6.6±1.5.Among them,279(27.9%)had good adherence,436(43.6%)had moderate adherence,and 286(28.6%)had poor adherence.The patients’medication adherence was related to age(P<0.05),times of medication(P<0.001),HIV transmission routes(P<0.05)and previous loss to ART(P<0.01).The patients who were≥50 years old and took medicine once a day had better adherence,whereas intravenous drug users and who had lost to ART in the past had worse adherence.Conclusion Different factors were related to medication adherence of HIV/AIDS patients and provide.Those with poor adherence should be included in HIV/AIDS case management early intervention to reduce the possibility of VL rebound and even loss to ART.
