环磷酰胺联合泼尼松治疗间质性肺炎的效果和不良反应

Effects and adverse reactions of cyclophosphamide combined with prednisone in treatment of interstitial pneumonia

目的:分析环磷酰胺联合泼尼松治疗间质性肺炎(IP)的效果和不良反应。方法:抽取2019年1月至2021年2月南阳市第二人民医院收治的IP患者108例,采用随机数字表法分为联合组和单一组,每组54例。单一组采用泼尼松,联合组采用泼尼松联合环磷酰胺。比较两组治疗效果、肺功能、血气指标、修正的医学研究委员会量表评分(mMRC)、临床-影像-生理(CRP)评分、炎性因子、不良反应。结果:联合组总有效率高于单一组( P<0.05);联合组治疗后肺活量、每分钟最大通气量高于单一组( P<0.05);联合组治疗后二氧化碳分压低于单一组,血氧分压高于单一组( P<0.05);联合组治疗后mMRC、CRP评分和涎液化糖链抗原-6)、肿瘤坏死因子-α水平低于单一组( P<0.05);两组不良反应比较差异未见统计学意义( P>0.05)。 结论:IP患者采用环磷酰胺联合泼尼松治疗,可减轻炎性刺激,改善血气指标、肺功能,减轻临床症状,且用药安全性好。

Objective To analyze the effects and adverse reactions of cyclophosphamide combined with prednisone in the treatment of interstitial pneumonia(IP).Methods A total of 108 patients with IP in the Second People’s Hospital of Nanyang from January 2019 to February 2021 were selected,and divided into combined group and single group by random number method,with 54 cases in each group.The single group was treated with prednisone,prednisone combined with cyclophosphamide was used in the combined group.The curative effect,pulmonary function,blood gas index,modified Medical Research Council(mMRC)dyspnea scale score,clinical rediographic physiologic(CRP)score,inflammatory factors and adverse reactions were compared between the two groups.Results The total effective rate of the combined group was higher than that of the single group(P<0.05).After treatment,vital capacity and maximum ventilation per minute in the combined group were higher than those in the single group(P<0.05).After treatment,partial pressure of carbon dioxide in the combined group was lower than that in the single group,and partial pressure of blood oxygenwas higher than that in the single group(P<0.05).The mMRC,CRP scores and the levels of salivary liquefied sugar chain antigen-6 and tumor necrosis factor-αin the combined group were lower than those in the single group(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusions The combined treatment of cyclophosphamide and prednisone in IP patients can reduce inflammatory stimulation,improve blood gas index and lung function,and relieve clinical symptoms,and have good drug safety.
