腹腔镜卵巢囊肿剥除术中单极电凝止血与缝合止血的效果比较

Comparison of unipolar electrocoagulation versus suture hemostasis in laparoscopic ovarian cyst stripping

目的比较腹腔镜卵巢囊肿剥除术中应用单极电凝止血、缝合止血的效果。方法选取河南大学淮河医院2020年1月至2021年6月收治的110例卵巢囊肿患者作为研究对象,按照随机数字表法分对照组与观察组,每组55例。两组卵巢囊肿患者均实施腹腔镜卵巢囊肿剥除术,其中研究组术中采用单极电凝止血,对照组术中行镜下缝合止血。记录两组手术情况及手术前、术后2个月经周期第2天卵巢功能相关指标检测值变化情况。结果两组术中操作所需时间及术中失血量比较,差异未见统计学意义(P>0.05)。两组卵巢囊肿患者腹腔镜卵巢囊肿剥除术前促卵泡激素(FSH)、雌二醇(E2)、促黄体生成素(LH)等卵巢功能相关指标比较,差异未见统计学意义(P>0.05),研究组术后2个月经周期第2天FSH、LH、E2等卵巢功能相关指标与对照组比较,差异有统计学意义(P<0.05)。结论针对腹腔镜卵巢囊肿剥除术中使用单极电凝止血可能对患者术后卵巢功能造成较大影响,经镜下缝合止血对患者术后卵巢功能影响较小,临床医生应跟进患者实际情况合理取舍腹腔镜卵巢囊肿剥除术中止血方式。

Objective To compare and analyze the effects of unipolar electrocoagulation and suture hemostasis on laparoscopic ovarian cyst stripping.Methods A total of 110 patients with ovarian cysts admitted to Huaihe Hospital of Henan University from January 2020 to June 2021 were selected as the subjects,and they were divided into the control group and the study group according to the random number table method,with 55 cases in each group.Both groups underwent laparoscopic ovarian cystectomy.The study group used monopolar electrocoagulation to stop bleeding during the operation,and the control group underwent endoscopic suture to stop bleeding during the operation.The operation status of the two groups and the changes in the detection values of ovarian function related indexes before and after the operation and on the 2 nd day of the 2 menstrual cycle were recorded.Results The intraoperative operation time and intraoperative blood loss between the two groups were not significant(P>0.05).There was no significant difference between the two groups in the detection values of ovarian function related indexes such as FSH,LH and E2 before laparoscopic ovarian cyst stripping(P>0.05).There was significant difference in the detection values of ovarian function related indexes such as FSH,LH and E2 on the second day of the two menstrual cycles in the study group compared with the control group(P<0.05).Conclusions For the use of unipolar electrocoagulation hemostasis in laparoscopic ovarian cyst stripping may have a great impact on patients’postoperative ovarian function,and suture hemostasis under microscope has little impact on patients’postoperative ovarian function,clinicians should follow up the actual situation of patients and reasonably choose the hemostasis method in laparoscopic ovarian cyst stripping.
