摘要
【目的】探讨血清微小RNA-98(miR-98)、白细胞介素-13(IL-13)在老年哮喘急性发作期患者血清中的表达水平及其临床意义。【方法】选取2017年3月至2020年1月在安康市中心医院确诊为老年哮喘患者360例,其中急性发作组200例(轻度组90例、中度组80例、重度组30例),缓解组160例;另外选取同期在安康市中心医院健康体检的老年人90例为健康组。采用实时荧光定量聚合酶链反应(Quantitative Real-time PCR,qRT-PCR)技术检测各组血清miR-98相对表达量,采用酶联免疫吸附测定法检测血清IL-13水平,采用肺功能仪检测急性发作组患者第一秒用力呼气量(FEV1)和最大呼气峰流速(PEF)并比较。【结果】急性发作组血清miR-98表达量显著低于缓解组和健康组,且随急性发作程度加重依次降低(P<0.05);血清IL-13表达水平高于缓解组和健康组,且随急性发作程度加重依次升高(P<0.05)。轻度组、中度组和重度组患者随着病情加重FEV1、PEF依次降低(P<0.05)。急性发作组患者血清中miR-98相对表达量与FEV1、PEF均呈正相关(P<0.05),与IL-13呈负相关(P<0.05);血清中IL-13的表达水平和FEV1、PEF均呈负相关(P<0.05)。【结论】哮喘急性发作期患者血清miR-98、IL-13呈异常表达,且均与病情发作程度和肺功能指标FEV1、PEF有关,可作为评估老年哮喘的潜在指标。
【Objective】To investigate the expression level and clinical significance of serum microrna-98(miR-98)and interleukin-13(IL-13)in elderly patients with acute attack of asthma.【Methods】A total of 360 elderly patients with asthma diagnosed in the Ankang Central Hospital from March 2017 to January 2020 was selected,including 200 cases in the acute attack group(90 cases in the mild group,80 cases in the moderate group and 30 cases in the severe group)and 160 cases in the remission group;In addition,90 elderly people who underwent physical examination in the Ankang Central Hospital in the same period were selected as the health group.The relative expression of serum miR-98 was detected by real-time fluorescence quantitative PCR(Quantitative Real-time PCR,qRT-PCR),the level of serum IL-13 was detected by enzyme-linked immunosorbent assay,and the first second forced expiratory volume(FEV1)and maximum peak expiratory flow rate(PEF)in acute attack group were detected and compared by pulmonary function instrument.【Results】The expression of serum miR-98 in the acute attack group was significantly lower than that in the remission group and the healthy group,and decreased successively with the aggravation of acute attack(P<0.05);The expression of serum IL-13 was higher than that in the remission group and the healthy group,and increased successively with the aggravation of acute attack(P<0.05).FEV1 and PEF in the mild group,the moderate group and the severe group decreased successively with the aggravation of the disease(P<0.05).The relative expression of miR-98 in serum of patients with the acute attack group was positively correlated with FEV1 and PEF(P<0.05),and negatively correlated with IL-13(P<0.05);The expression level of IL-13 in serum was negatively correlated with FEV1 and PEF(P<0.05).【Conclusion】The abnormal expression of serum miR-98 and IL-13 in patients with acute attack of asthma is related to the degree of attack and the indexes of pulmonary function FEV1 and PEF.It can be used as a potential index to evaluate the elderly asthma.
