盐酸奥布卡因凝胶对中老年男性留置导尿管患者苏醒期躁动及尿道刺激的影响

Effect of oxybuprocaine hydrochloride gel on emergence?agitation and urethral stimulation in middle-aged and elderly male patients with indwelling catheter

目的观察盐酸奥布卡因凝胶对中老年男性患者围术期留置导尿管引起苏醒期躁动及尿道刺激的影响。方法纳入留置导尿管的中老年(50岁以上)男性全麻手术患者44例,随机分为利多卡因胶浆组(22例)和盐酸奥布卡因凝胶组(22例)。所有患者均在麻醉诱导后行尿管置入,分别用利多卡因胶浆和盐酸奥布卡因凝胶涂抹尿管和尿道,分析比较2组患者围术期及苏醒期MAP、HR和SpO_(2)变化,苏醒期躁动及尿道刺激的情况。结果2组患者苏醒期MAP和SpO_(2)比较差异无统计学意义(P>0.05),与奥布卡因凝胶组相比,利多卡因胶浆组患者在入PACU和出PACU时HR明显增快(P<0.05);盐酸奥布卡因凝胶组患者苏醒期躁动评分及尿道刺激的发生率明显低于利多卡因胶浆组(P<0.05)。结论盐酸奥布卡因凝胶用于中老年男性患者全麻手术留置导尿管术可减少苏醒期躁动及尿道刺激的发生率,有利于患者的术后恢复,提高麻醉质量。

Objective To observe the effect of oxybuprocaine hydrochloride gel on emergence agitation and urethral stimulation in middle-aged and elderly male patients with indwelling catheter.Methods A total of 44 male patients(over 50 years old)undergoing general anesthesia were randomly divided into the lidocaine gel group(n=22)and the oxybuprocaine hydrochloride gel group(n=22).All patients were given catheterization after induction of anesthesia.Lidocaine gel and Oxybuprocaine hydrochloride gel were applied to the urethra.The changes of MAP,HR and SpO_(2) and the emergence agitation and the urethral stimulation were analyzed and compared between the two groups.Results There was no significant difference in MAP and SpO_(2) between the two groups(P>0.05).Compared with the oxybuprocaine group,HR in lidocaine group was significantly increased when entering and leaving PACU(P<0.05).The incidence of emergence agitation and urethral stimulation in the oxybuprocaine hydrochloride gel group were significantly lower than those in lidocaine gel group(P<0.05).Conclusion The application of oxybuprocaine hydrochloride gel in indwelling catheterization for elderly male patients can reduce the incidence of emergence agitation and the symptoms of urethral stimulation,which is beneficial to the postoperative recovery and improve the quality of anesthesia.
