小V型切口Treitz肌保存术联合PPH治疗混合痔的效果

Efficacy of Treitz muscle preservation with small V-incision combined with PPH in the treatment of mixed hemorrhoids

目的研究小V型切口Treitz肌保存术联合吻合器痔上黏膜环切术(PPH)治疗混合痔的效果。方法选择2017年6月至2021年1月在安徽省宣城市人民医院就诊的80例混合痔患者为研究对象,根据随机数表法将其分为研究组和对照组,每组各40例。对照组采用PPH治疗,研究组在PPH基础上加用小V型切口Treitz肌保存术治疗。分析比较两组临床有效率、肛门坠胀、肛门疼痛、排尿状况、并发症发生率及安全性。结果研究组临床疗效优于对照组,差异有统计学意义(P<0.05)。术后24 h,两组肛门坠胀程度、疼痛程度及排尿情况评分均明显低于术后6 h,差异有统计学意义(P<0.05)。术后6、24 h,研究组肛门坠胀程度、疼痛程度及排尿情况评分均明显低于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率明显低于对照组,差异有统计学意义(P<0.05)。两组未发生恶心呕吐、眩晕呼吸不畅、肝肾功能异常等不良反应,安全性较好。结论小V型切口Treitz肌保存术联合PPH治疗混合痔临床疗效较好、能够有效降低术后肛门坠胀和疼痛情况,是一种安全有效的治疗方式。

Objective To study the effect of Treitz muscle preservation with small V-incision combined with procedure for prolapse and hemorrhoids(PPH)in the treatment of mixed hemorrhoids.Methods A total of 80 patients with mixed hemorrhoids admitted to the People’s Hospital of Xuancheng City of Anhui Province from June 2017 to January 2021 were selected as the research subjects,and they were divided into study group and control group according to random number table method,with 40 patients in each group.The control group was treated with PPH,and the research group was treated with Treitz muscle preservation with small V-incision on the basis of PPH.The clinical effective rate,anal distention,anal pain,urination status,incidence of complications and safety were analyzed and compared between the two groups.Results The clinical efficacy of the study group was better than that of the control group,and the difference was statistically significant(P<0.05).Twenty-four h after surgery,the degree of anal distension,pain,and urination status score of both groups were significantly lower than those of 6 h after surgery,the differences were statistically significant(P<0.05).At 6 and 24 h after surgery,the degree of anal distension,pain and urination status score in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of complications in the study group was significantly lower than that in the control group,the differences were statistically significant(P<0.05).There were no adverse reactions such as nausea and vomiting,dizziness and obstructive breathing,abnormal liver and kidney function,and the safety was good in the two groups.Conclusion Treitz muscle preservation with small V-incision combined with PPH has good clinical efficacy in the treatment of mixed hemorrhoids and can effectively reduce postoperative anal distension and pain,which is a safe and effective treatment.
