小金丸联合夏枯草胶囊治疗桥本甲状腺炎伴结节的效果及对炎症因子的影响

Effect of Xiaojin Pills combined with Xiakucao Capsules in the treatment of Hashimoto thyroiditis with nodules and its influence on inflammatory factors

目的探讨小金丸联合夏枯草胶囊治疗桥本甲状腺炎伴结节的效果及对炎症因子的影响。方法选取2018年5月至2020年6月赣南医学院第三附属医院收治的60例桥本甲状腺炎伴结节患者作为研究对象,采用随机数字表法分为对照组(30例)与研究组(30例)。对照组采用左甲状腺素钠片,研究组患者在对照组基础上采用小金丸及夏枯草胶囊。12周为1个疗程,两组患者均连续治疗2个疗程。比较两组患者治疗前及治疗12、24周的甲状腺体积与甲状腺结节大小、甲状腺功能指标、炎症因子水平及治疗期间的不良反应。结果研究组患者治疗12、24周后的甲状腺体积及甲状腺结节直径小于对照组,差异有统计学意义(P<0.05);研究组患者治疗12、24周后的血清抗甲状腺过氧化物酶抗体(TPO-Ab)、抗甲状腺球蛋白抗体(TG-Ab)、促甲状腺激素(TSH)、γ干扰素(IFN-γ)、白介素-2(IL-2)水平均低于对照组,差异有统计学意义(P<0.05);研究组患者治疗12、24周后的血清白介素-10(IL-10)水平均高于对照组,差异有统计学意义(P<0.05);治疗期间研究组患者的不良反应总发生率为3.33%,低于对照组患者的23.33%,差异有统计学意义(P<0.05)。两组治疗前的甲状腺体积、甲状腺结节直径与血清TPO-Ab、TG-Ab、TSH、IFN-γ、IL-2、IL-10水平比较,差异无统计学意义(P>0.05)。结论小金丸联合夏枯草胶囊能够缩小桥本甲状腺炎伴结节患者的甲状腺体积及结节大小,改善甲状腺抗体水平,提高甲状腺功能,抑制炎症反应,同时不良反应较少。

Objective To investigate the effect of Xiaojin Pills combined with Xiakucao Capsules in the treatment of Hashimoto thyroiditis with nodules and its influence on inflammatory factors.Methods A total of 60 patients with Hashimoto thyroiditis with nodules who were admitted to the Third Affiliated Hospital of Gannan Medical College from May 2018 to June 2020 were selected as the research subjects.They were divided into the control group(n=30)and the study group(n=30)according to random number table method.The patients in control group were treated with Levothyroxine Sodium tablets,and the patients in the study group were treated with Xiaojin Pills and Xiakucao Capsules on the basis of the control group.The course of treatment lasted 12 weeks,for two consecutive courses.The thyroid volume and thyroid nodule size,thyroid function indexes,inflammatory factor levels before treatment and at 12 and 24 weeks of treatment,and adverse reactions between the two groups were compared.Results The thyroid volume and the diameter of thyroid nodules in the study group after 12 and 24 weeks of treatment were smaller than those in the control group,and the differences were statistically significant(P<0.05).The levels of serum antithyroid peroxidase antibody(TPO-Ab),anti-thyroglobulin antibody(TG-Ab),thyroid stimulating hormone(TSH),γ-interferon(IFN-γ)and interleukin-2(IL-2)in the study group after 12 and 24 weeks of treatment were lower than those in the control group,with statistical significances(P<0.05).The level of serum interleukin-10(IL-10)of patients in the study group after 12 and 24 weeks of treatment were higher than those in the control group,and the differences were statistically significant(P<0.05).During the treatment period,the total incidence of adverse reactions in the study group was 3.33%,which was lower than that of the control group accounting for 23.33%,with statistical significance(P<0.05).There were no significant differences in the thyroid volume,thyroid nodule diameter,and serum levels of TPO-Ab,TG-Ab,TSH,IFN-γ,IL-2,and IL-10 before treatment(P>0.05).Conclusion Xiaojin Pills combined with Xiakucao Capsules can reduce the thyroid volume and nodule size in patients with Hashimoto thyroiditis with nodules,improve the level of thyroid antibodies,enhance thyroid function,inhibit inflammation,and have few adverse reactions.
