多排螺旋CT在肋骨骨折诊断中的效果和影像学表现

Effect and imaging findings of multi-slice spiral CT in the diagnosis of rib fracture

目的探讨多排螺旋CT在肋骨骨折诊断中的效果和影像学表现。方法选取2019年1月至2021年1月山东第一医科大学附属省立医院收治的100例疑似肋骨骨折患者作为研究对象,依次采用多排螺旋CT、数字化摄影DR诊断对患者进行检查,以临床手术诊断结果作为金标准,比较不同诊断措施的特异度、灵敏度、准确度、并发症的诊断情况以及影像学表现。结果100例疑似肋骨骨折患者中,有87例肋骨骨折经临床最终诊断确诊。多排螺旋CT诊断的灵敏度、特异度、准确度均高于数字化摄影DR检查,差异有统计学意义(P<0.05);两种方法的阳性预测值比较,差异无统计学意义(P>0.05),多排螺旋CT诊断气胸、肺挫伤并发症检出率高于数字化摄影DR检查,差异有统计学意义(P<0.05)。结论针对肋骨骨折患者,多排螺旋CT和数字化摄影DR检查均具有临床诊断价值,但是多排螺旋CT技术具有更高的诊断准确性,可更加直接地反映患者的骨折情况,值得推荐。

Objective To investigate the effect and imaging manifestations of multi-slice spiral CT in the diagnosis of rib fractures.Methods A total of 100 patients with suspected rib fractures admitted to the Provincial Hospital of Shandong First Medical University from January 2019 to January 2021 were selected as the research objects,and the patients were examined by multi-slice spiral CT and digital photographic DR diagnosis.The clinical surgical diagnosis results was used as the gold standard to compare the specificity,sensitivity,accuracy,diagnosis of complications,and imaging performance of different diagnostic measures.Results Among 100 patients with suspected rib fractures,87 cases of rib fractures were confirmed by clinical final diagnosis.The sensitivity,specificity,and accuracy of CT diagnosis were higher than those of digital photographic DR examination,and the differences were statistically significant(P<0.05).There was no statistically significant difference in positive predictive value between the two methods(P>0.05).The detection rate of spiral CT in the diagnosis of pneumothorax and pulmonary contusion were higher than those of digital photographic DR examination,and the differences were statistically significant(P<0.05).Conclusion For patients with rib fractures,both multi-slice spiral CT and digital photographic DR examination have clinical diagnostic value,but multi-slice spiral CT technology has higher diagnostic accuracy and can provide more direct feedback on the fracture status of patients.The program is worth recommending.
