以短暂性脑缺血发作为表现的凸面蛛网膜下腔出血1例报道及文献学习

Convex subarachnoid hemorrhage presented by transient cerebral ischemia:a case report and literature study

凸面蛛网膜下腔出血(cSAH)以单纯的蛛网膜下腔出血为特征,其病因及临床表现多种多样,以短暂性脑缺血发作为其临床表现的并不多见。且大部分cSAH病例报道发现其责任血管狭窄或闭塞位于前循环,而cSAH的责任血管出现在后循环的较少见。本研究病例是以短暂性脑缺血发作为表现的cSAH,该患者为基底动脉狭窄或闭塞伴左侧顶叶cSAH。该患者经稳定斑块治疗后临床病情稳定。该病例提示基底动脉的狭窄或闭塞可能会引起cSAH的形成。虽然这种病因目前尚未达成共识,但该患者未出现不良预后。

Convexal subarachnoid hemorrhage(cSAH)is characterized by simple subarachnoid hemorrhage.The etiology and clinical manifestations of cSAH are varied,and transient cerebral ischemia is rare.In addition,the most cases of cSAH reported stenosis or occlusion of responsible vessels in the anterior circulation,while the presence of responsible vessels in the posterior circulation of cSAH was rare.The case mentioned in this study was cSAH presented by transient cerebral ischemia,and the patient was basilar artery stenosis or occlusion with left parietal cSAH.The patient was clinically stable after treatment with stable plaque.This case suggests that narrowing or occlusion of the basilar artery may cause cSAH formation.Although the etiology of this disease has not been determined,the patient did not have an adverse prognosis.
