摘要
目的探讨阻塞性睡眠呼吸暂停综合征(OSAHS)对肥胖儿童健康的早期危害,为肥胖儿童睡眠障碍的临床防治及基础研究提供理论依据。方法对兰州市4个区小学生进行体格检查和睡眠问卷调查,选出符合OSAHS和肥胖标准的儿童80例作为研究对象,既无睡眠障碍也无肥胖的健康儿童51例作为对照组。结果肥胖伴OSAHS的儿童腹围、腹围/身高和血压均高于对照组,且腹围、腹围/身高和血压呈正相关关系,均有显著性差异(P<0.01);OSAHS肥胖儿童心电图发生异常的比率高于对照组(P<0.05),中、重度组儿童体块指数高于轻度组;肥胖儿童体块指数与呼吸暂停低通气指数呈正相关关系,与动脉血氧饱和度呈负相关关系,差异有统计学意义(P<0.01);OSAHS肥胖儿童的代谢指标高于对照组。结论腹型肥胖儿童会引起血压升高,肥胖儿童OSAHS严重程度与体块指数相关。OSAHS肥胖儿童存在早期肝损害和代谢紊乱,是成年后发生健康问题的潜在威胁人群。
Objective To explore the early health hazards in obese obstructive sleep apnea hypopnea syndrome(OSAHS)children,to provide a scientific theoretical basis for clinical prevention and basic research into obese children with sleep disorders.Methods During the physical examination and sleep questionnaire survey of pupils in 4 districts of Lanzhou City,80 children who met the criteria of OSAHS and obesity were selected as the study subjects for sleep monitoring.51 healthy children with age and gender matching without sleep disorders or obesity as the control group.Results The abdominal circumference,abdominal circumference/height and blood pressure of children with obesity and OSAHS were higher than those of the control group,the abdominal circumference,abdominal circumference/height and blood pressure of obese children with OSAHS were positively correlated(P<0.01).The rate of abnormal electrocardiogram in obesity children with OSAHS was higher than that in the control group(P<0.05).In the OSAHS obese children,the moderate to severe body mass index and apnea-hypopnea index(AHI)were higher than that of the mild group,while the oxygen saturation in arterial bloocl(SaO2)was lower than that of the mild group.Obesity children’s body mass index was positively correlated with AHI and negatively correlated with SaO2.The metabolic index of OSAHS obesity children was higher than that of the control group.Conclusion The severity of OSAHS in children is related to the body mass index.OSAHS obesity children undergo early liver damage and metabolic disorders,which pose potential threats to their health.
