中医体质辨识对慢性心力衰竭患者生活质量及满意度的影响

Effect of Traditional Chinese Medicine Constitution Identification on Quality of Life and Satisfaction of Patients with Chronic Heart Failure

目的观察研究中医体质辨识对慢性心力衰竭患者生活质量及满意度的影响。方法纳入符合研究标准的慢性心力衰竭患者120例,随机分为试验组和对照组各60例。2组患者均进行常规的中西医治疗,在此基础上对照组给予常规护理指导,试验组应用舌面脉信息采集中医体质辨识系统及中医体质标准化量表收集患者生理、心理信息,判别慢性心力衰竭患者的体质类型,根据体质类型的不同进行具有中医特色的、个性化的护理指导。比较2组患者的护理满意度,跟踪随访3个月后比较2组患者干预后明尼苏达心力衰竭生活质量问卷评分(MLHF)。结果干预后临床护理满意度试验组优于对照组(P<0.05)。跟踪随访3个月后,2组明尼苏达心力衰竭生活质量问卷评分(MLHF)均较治疗前降低,且试验组低于对照组(均P<0.05)。结论对慢性心力衰竭患者应用中医体质辨识理论进行护理干预,更具有中医特色,更有针对性,依从性更高,可明显提高患者满意度,有效改善生活质量。

Objective To observe the clinical effect of traditional Chinese medicine(TCM)constitution identification on the quality of life and satisfaction of patients with chronic heart failure.Methods 120 patients with chronic heart failure were randomly divided into experimental group and control group with 60 cases in each group.Both groups were treated with traditional Chinese and western medicine.On this basis,the control group was given routine nursing guidance.The experimental group used tongue and facial pulse information collection system and TCM constitution standardized scale to collect the physiological and psychological information of patients to identify the constitution type of patients with chronic heart failure,and to carry out personalized nursing guidance with Chinese medicine characteristics according to the different constitution types.The nursing satisfaction of the two groups was compared,and the scores of Minnesota heart failure quality of life questionnaire(MLHF)after 3 months of follow-up were compared between the two groups.Results After the intervention,the clinical nursing satisfaction of the experimental group was better than that of the control group(P<0.05).After 3 months of follow-up,the scores of MLHF in the two groups were lower than those before treatment,and the experimental group was lower than the control group(all P<0.05).Conclusion The application of TCM constitution identification theory for nursing intervention in patients with chronic heart failure has more characteristics of traditional Chinese medicine,more targeted,higher compliance,can significantly improve patient satisfaction and effectively improve the quality of life.
