自拟化瘀通络汤联合西药治疗糖尿病足临床观察

Clinical Observation on Self-made Huayu Tongluo Decoction Combined with Western Medicine in the Treatment of Diabetic Foot

目的综合性评估化瘀通络汤联合西医治疗糖尿病足的临床效果。方法选取2018年6月—2020年6月于抚顺市中医院内分泌科病房住院并接受治疗的糖尿病足患者60例,入组病例均采用信封法随机分为对照组和试验组。对照组30例,应用西医内、外科常规治疗方案;试验组30例,在对照组基础上联合化瘀通络汤内服治疗方案。观察2组患者治疗前后中医证候积分、踝臂血压指数及下肢多普勒超声积分变化情况。结果经临床治疗,试验组中医证候评分(9.35±3.12)分,踝臂血压指数(0.95±0.14)分,下肢血管多普勒超声积分(6.01±0.33)分,对照组中医证候评分(12.32±3.48)分,踝臂血压指数(0.86±0.15)分,下肢血管多普勒超声积分(6.42±0.32)分,试验组均优于对照组,差异有统计学意义(P<0.05)。结论化瘀通络汤联合西医、内外科常规治疗方案可有效改善患者下肢动静脉血流动力学水平,促进创面愈合、改善微循环,益气扶正、化瘀通络、祛腐生新,效果显著,值得推广。

Objective To comprehensively evaluate the clinical effects of Huayu Tongluo decoction combined with conventional western medicine treatment on diabetic foot patients.Methods From June 2018 to June 2020,60 diabetic foot patients in Fushun Hospital of Traditional Chinese Medicine were selected.The enrolled cases were randomly divided into control group and experimental group by envelope method.Control group(30 cases)received conventional medical and surgical treatment.Experimental group of 30 cases,on the basis of the control group,combined with Huayu Tongluo decoction.The changes of traditional Chinese medicine(TCM)syndrome score,ankle brachial blood pressure index and lower limb Doppler ultrasound score were observed before and after treatment in 2 groups.Results After clinical treatment,TCM syndrome score of experimental group(9.35±3.12)points,ankle brachial blood pressure index(0.95±0.14)points,lower limb vascular Doppler ultrasound score(6.01±0.33)points,TCM syndrome score of control group(12.32±3.48)points,ankle brachial blood pressure index(0.86±0.15)points,Lower limb vascular Doppler ultrasound score(6.42±0.32)was better in experimental group than in control group,and the difference was statistically significant(P<0.05).Conclusion Huayu Tongluo decoction combined with conventional Western medicine internal medicine and surgical treatment can effectively improve the patient's lower limb arteriovenous hemodynamics.The combination of traditional Chinese and western programs can be used to lower lipids,remove blood stasis It has significant clinical effects and is worthy of clinical promotion.
