基于数据挖掘探讨喻斌治疗胃食管反流病用药规律

Medication rule of Yu Bin in treatment of gastroesophageal reflux disease based on data mining

目的:基于数据挖掘分析喻斌教授治疗胃食管反流病(GERD)的用药规律。方法:收集2020年4月至2021年4月在喻斌教授门诊处就诊的GERD病案,将其相关信息资料录入中医传承辅助平台(V2.5),对药物进行四气、五味、归经统计分析,用药频次分析,组方规律及新方分析。结果:共纳入病案200则,涉及处方265首,中药132味。药物药性以寒、温为主;药味以苦、辛、甘居多;归经多属于胃、脾、肺经;使用频率>10%的药物共41味,其中排名前5位的依次为甘草、黄连、厚朴、法半夏、茯苓;挖掘到3个新处方,处方1:竹茹-石菖蒲-枳实-白芍-旋覆花-柴胡-陈皮;处方2:石膏-木香-白术-知母;处方3:合欢皮-麦冬-百合-牡蛎-远志。结论:喻教授治疗GERD寒温并用,辛开苦降,佐以甘缓,以半夏厚朴汤合小陷胸汤为基础,重视从肺胃论治,兼以治肝,调养心神,顾护阴液。

Objective:To investigate the medication rule of Professor Yu Bin in the treatment of gastroesophageal reflux disease(GERD)based on data mining.Methods:Medical records of GERD were collected from the patients who attended the outpatient service of Professor Yu Bin from April 2020 to April 2021.Related data were entered into Traditional Chinese Medicine Inheritance Support System(V2.5),and statistical analyses were performed for flavors,tastes,and meridian tropism.Frequency of medication,prescription rule,and new prescriptions were also analyzed.Results:A total of 200 medical records were included,involving 265 prescriptions and 132 traditional Chinese medicine(TCM)drugs.Most drugs were cold or warm in nature,and most drugs had a bitter,pungent,or sweet taste.As for meridian tropism,most drugs entered the stomach,spleen,and lung meridians.There were 41 drugs with a frequency of>10%,and the top 5 drugs were Radix Glycyrrhizae,Coptis chinensis,Magnolia officinalis,Rhizoma Pinelliae Praeparatum,and Poria cocos.Three new prescriptions were obtained,i.e.,prescription 1 of Bambusae Caulis in Taenias-Rhizoma Acori graminei-Fructus Aurantii Immaturus-Radix Paeoniae Alba-inula flower-Bupleurum chinense-tangerine peel,prescription 2 of Gypsum Fibrosum-Saussurea costus-Atractylodes macrocephala Koidz.-Rhizoma Anemarrhenae,and prescription 3 of Cortex Albiziae-Radix Ophiopogonis-lily-Concha Ostreae-Polygala tenuifolia.Conclusion:In the treatment of GERD,Professor Yu uses both cold and warm drugs and achieves the effect of lifting with pungent herbs and descending with bitter herbs,with the help of sweet herbs.Banxia Houpo decoction combined with Xiaoxianxiong decoction is the basic treatment,and Professor Yu emphasizes the treatment from lung and stomach,considers the treatment of the liver at the same time,regulates the mind,and takes care of Yin-fluid.
