结肠癌术前多层螺旋CT增强扫描结合血管造影的评估价值

Evaluation of preoperative multi-slice spiral CT enhanced scanning combined with angiography in colon cancer

目的:分析结肠癌术前采用多层螺旋CT增强扫描结合血管造影的评估价值,探讨一种有效的诊断方法,为临床提供参考依据。方法:选取济宁市兖州区中医医院2020年2月—2021年2月诊治的62例结肠癌患者,所有患者术前均行多层螺旋CT增强扫描与血管造影检查,以手术病理结果为标准,分析多层螺旋CT增强扫描结合血管造影检查对结肠癌分期、CT征象、累及与侵犯相邻血管情况的符合率。结果:多层螺旋CT增强扫描结合血管造影检查对结肠癌分期与手术病理结果的符合率为83.87%。多层螺旋CT增强扫描结合血管造影检查CT征象中肿块边缘不规则、肠壁增厚肿胀、肠腔狭窄、累及浆膜层、肠梗阻、累及相邻组织脏器、腹膜后淋巴结转移的符合率分别为96.77%、95.00%、90.32%、88.00%、100.00%、85.71%、92.31%;检查累及与侵犯腹主动脉、下腔静脉、肾动脉、肾静脉、脾动脉、脾静脉、肠系膜上动脉、肠系膜上静脉、肠系膜下动脉、肠系膜下静脉的符合率分别为100.00%、80.00%、77.78%、88.89%、100.00%、80.00%、82.35%、89.47%、86.67%、85.00%。结论:多层螺旋CT增强扫描结合血管造影在结肠癌术前检查中的应用价值较高,可较为准确地评估患者病情,有助于指导手术方案的制定,值得推行。

Objective To analyze the evaluation value of preoperative multi-slice spiral CT enhanced scanning combined with angiography for colon cancer,and to explore an effective diagnostic method to provide clinical reference.Methods 62 patients with colon cancer treated in Yanzhou District Hospital of Traditional Chinese Medicine from February 2020 to February 2021 were selected.All patients underwent multi-slice spiral CT enhanced scanning and angiography before the operation.The pathological results of the operation were used as the standard to analyze the coincidence rate of enhanced multi-slice spiral CT scan combined with angiography for colon cancer staging,CT signs,involvement and invasion of adjacent blood vessels.Results The coincidence rate of enhanced multi-slice spiral CT scan combined with angiography on colon cancer staging and surgical pathology results was 83.87%.Multi-slice spiral CT enhanced scanning combined with angiography showed that the edges of the tumor were irregular,the intestinal wall was thickened and swollen,the intestinal cavity was narrow,the serosal layer was involved,the intestinal obstruction,and the neighboring organs were involved.The coincidence rates of the detection of retroperitoneal lymph node metastasis were 96.77%,95.00%,90.32%,88.00%,100.00%,85.71%,92.31%;The coincidence rates of involving abdominal aorta,inferior vena cava,renal artery,and renal vein The coincidence rates of detection of splenic artery,splenic vein,superior mesenteric artery,superior mesenteric vein,inferior mesenteric artery,and inferior mesenteric vein were 100.00%,80.00%,77.78%,88.89%,100.00%,80.00%,82.35%,89.47%,86.67%,85.00%.Conclusion Multi-slice spiral CT enhanced scan combined with angiography has high application value in the preoperative examination of colon cancer.It can more accurately assess the patient’s condition,help guide the formulation of the surgical plan,and is worthy of implementation.
