中西医结合治疗脾肾阳虚型膝骨关节炎临床研究

Clinical Study on Combination of Traditional Chinese and Western Medicine in the Treatment of Knee Osteoarthritis of Yang Deficiency of Spleen and Kidney

目的观察中西结合治疗脾肾阳虚型膝骨关节炎的临床疗效及对膝关节相关炎症因子的影响。方法选取2018年5月—2019年12月沈阳市骨科医院确诊为脾肾阳虚型膝骨关节炎患者68例,随机分成对照组和医治组各34例,对照组口服塞莱昔布胶囊治疗,2次/d,100 mg/次,治疗8周。医治组在对照组基础上口服中药治疗,8周后比较2组疗效,VAS、WOMAC评分及白细胞介素-1β(IL-1β)、白细胞介素-17(IL-17)、血沉(ESR)和C反应蛋白(CRP)的水平。结果医治组的治疗总有效率为97.06%,明显高于对照组的85.29%(P<0.05);医治组VAS、WOMAC评分、IL-1β、IL-17、ESR、CRP水平均较对照组下降的更明显(P<0.05)。结论采用中西医结合治疗脾肾阳虚型膝骨关节炎,可有效减轻患者临床表现,缓解患者疼痛,并降低体内炎症相关因子。

Objective To observe the clinical effect of combination traditional Chinese and Western in the treatment of knee osteoarthritis of yang deficiency of spleen and kidney and its influence on knee joint-related inflammatory factors.Methods A total of 68 patients with knee osteoarthritis diagnosed with spleen and kidney yang deficiency in Shenyang Orthopedic Hospital from May 2018 to December 2019 were selected and randomly divided into control group and treatment group,with 34 cases in each.The control group was treated with celecoxib capsules orally,2 times/d,100 mg/time,treatment for 8 weeks.On the basis of the control group,the treatment group was treated with the self-made prescription of our hospital.After 8 weeks,the two groups were compared the efficacy,VAS,WOMAC scores,interleukin-1β(IL-1β),interleukin-17(IL-17),Erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP) levels.Results The total effective rate of treatment in the treatment group was 97.06%,which was significantly higher than 85.29% in the control group,(P< 0.05).The VAS,WOMAC score,IL-1β,IL-17,ESR,and CRP levels in the treatment group were higher than those in the control group.The decrease was more obvious(P< 0.05).Conclusion The use of integrated traditional Chinese and western medicine to treat knee osteoarthritis of spleen and kidney yang deficiency type can effectively reduce the patient’s clinical manifestations,relieve the patient’s pain and knee joint ability,and reduce inflammation-related factors in the body.
